Tóm tắt

Ung thư thực quản, nguyên nhân thứ sáu phổ biến nhất gây tử vong do ung thư trên toàn thế giới, bao gồm các loại mô học khác nhau và thể hiện các mẫu tỷ lệ mắc bệnh đa dạng. Ung thư biểu mô tuyến thực quản và ung thư biểu mô tế bào vảy thực quản là hai loại phổ biến nhất. Các nghiên cứu dịch tễ học đã chỉ ra rằng việc tiêu thụ các chất nóng là một yếu tố nguy cơ chính đối với ung thư biểu mô tế bào vảy, do đó việc đánh giá tác động của áp lực bên ngoài này lên tế bào thực quản là điều cần thiết. Loại áp lực này gây ra những thay đổi tế bào và làm ổn định chúng bằng cách ảnh hưởng đến các đặc điểm tế bào khác nhau như tính ổn định di truyền, tính toàn vẹn của màng và sự điều hòa của các con đường tín hiệu. Nó cũng gây tổn thương mô bằng cách ảnh hưởng đến ma trận ngoại bào và khả năng sống sót của tế bào. Do đó, một trong những hậu quả chính của tổn thương nhiệt là sự kích hoạt hệ thống miễn dịch, có thể dẫn đến viêm mãn tính. Sự thay đổi di truyền xảy ra trong quá trình tổn thương nhiệt và sự giảm chức năng của các hệ thống sửa chữa được củng cố thêm bởi viêm mãn tính, do đó tăng khả năng xuất hiện các dòng tế bào đột biến. Các phân tử có mặt trong hoàn cảnh này, chẳng hạn như protein sốc nhiệt, cytokine, chemokine và các yếu tố viêm khác, ảnh hưởng đến các con đường tín hiệu liên tế bào, bao gồm yếu tố nhân kappa‐light‐chain‐enhancer của tế bào B hoạt hóa, người truyền đạt tín hiệu kích hoạt phiên mã‐3 và yếu tố gây thiếu oxy 1α trong việc hỗ trợ sự sống sót và sự xuất hiện của các kiểu hình đột biến cũng như sự tiến triển ác tính tiếp theo trong các dòng tế bào bị thay đổi. Cuộc điều tra về các yếu tố hiệu quả này và vai trò có thể của chúng trong con đường sinh ung thư có thể cải thiện hiểu biết hiện tại.
