Tỷ lệ tiếp cận xét nghiệm HIV và virus viêm gan C tại các phòng khám methadone ở miền Nam Trung Quốc: cơ hội mở rộng phát hiện các nhiễm trùng qua đường máu

Sự đồng nhiễm HIV và viêm gan C (HCV) rất phổ biến trong số những người tiêm chích ma túy (IDUs) ở Trung Quốc, nhưng rất khó để tiếp cận các IDUs tại các phòng khám tư vấn điều trị HIV (VCT) truyền thống. Một mô hình quốc gia mới tích hợp xét nghiệm HIV/HCV với điều trị duy trì methadone đã được bắt đầu vào năm 2006. Mục đích của nghiên cứu này là điều tra tỷ lệ tiếp cận xét nghiệm HIV và HCV cùng các yếu tố liên quan tại các phòng khám methadone ở tỉnh Quảng Đông, Trung Quốc. Thiết kế nghiên cứu cắt ngang sử dụng dữ liệu giám sát thường xuyên và xác nhận xét nghiệm tại phòng thí nghiệm đã được áp dụng để xác định tỷ lệ tiếp cận xét nghiệm HIV và HCV. Mô hình đa mức đã được sử dụng để kiểm tra các yếu tố tương quan ở cấp độ cá nhân và cấp độ phòng khám liên quan đến việc tăng cường tỷ lệ tiếp cận xét nghiệm. 45 trong số 49 phòng khám methadone tại tỉnh Quảng Đông đã đồng ý tham gia nghiên cứu. Trong số 13.270 cá nhân, có 10.046 người (75,7%) có tỷ lệ tiếp cận xét nghiệm HIV và 10.404 người (78,4%) có tỷ lệ tiếp cận xét nghiệm HCV. Tại cấp độ cá nhân, những khách hàng methadone từ 30 tuổi trở lên có khả năng cao hơn để thực hiện xét nghiệm HIV và HCV (p <0.001 cho cả hai). Tại cấp độ phòng khám, những phòng khám methadone có nhiều nhân viên chăm sóc sức khỏe hơn có khả năng cao có tỷ lệ tiếp cận xét nghiệm HIV (p =0.01) và HCV (p = 0.044). Tỷ lệ tiếp cận xét nghiệm HIV có mối tương quan đáng kể với tỷ lệ tiếp cận xét nghiệm HCV (hệ số tương quan=0.64). Các phòng khám methadone cung cấp cơ hội cho việc sàng lọc xét nghiệm HIV và HCV tích hợp thường xuyên trong số những người sử dụng ma túy ở Trung Quốc. Tăng cường tiếp cận xét nghiệm ở những người sử dụng ma túy trẻ tuổi và tăng số lượng nhân viên chăm sóc sức khỏe tại các phòng khám có thể cải thiện hơn nữa việc sàng lọc.
