Bệnh Griscelli: Mối tương quan giữa kiểu gen và kiểu hình trong một loạt các hình thái lâm sàng khác nhau

Bệnh Griscelli là một rối loạn hiếm gặp di truyền tự nhiên lặn, đặc trưng bởi sự pha loãng sắc tố lan tỏa và sự xuất hiện các giai đoạn cấp tính của sự kích hoạt không kiểm soát của bạch cầu lympho và đại thực bào, được gọi là "hội chứng huyết thực" (HS), dẫn đến tử vong. Gần đây, hai gen liên kết chặt chẽ nằm trên vùng 15q21 của con người đã được xác định là nguyên nhân gây ra bệnh. Chúng tôi trình bày các phát hiện lâm sàng và xét nghiệm của 13 bệnh nhân không có mối quan hệ huyết thống bị bệnh Griscelli cũng như các phân tích đột biến nhằm xác định mối tương quan giữa kiểu gen và kiểu hình. Tám bệnh nhân đã cho thấy đột biến RAB27A có biểu hiện triệu chứng HS. Ngược lại, hai bệnh nhân chủ yếu có biểu hiện suy giảm thần kinh mà không có sự nhạy cảm với nhiễm trùng hoặc HS đã được phát hiện có đột biến MYO5A đồng hợp tử. Không phát hiện đột biến nào ở RAB27A ở ba bệnh nhân còn lại. Một trong số họ phát triển HS ở độ tuổi khá muộn, trong khi hai bệnh nhân còn lại không có HS ở độ tuổi lần lượt là 12 và 15. Bệnh Griscelli thể hiện một bức tranh lâm sàng không đồng nhất dường như phản ánh khiếm khuyết gen liên quan. Mối tương quan giữa kiểu gen và kiểu hình này gợi ý rằng diễn biến tự nhiên của bệnh và kết quả là do vị trí và loại đột biến di truyền.
