Trong nephrology lâm sàng, tỷ lệ lọc cầu thận (GFR) đã được công nhận là tiêu chuẩn vàng để đánh giá chức năng thận. Tuy nhiên, GFR bình thường không nhất thiết có nghĩa là khả năng lọc của thận bình thường, vì khả năng lọc bị suy giảm có thể được bù đắp bằng cách tăng áp lực thủy tĩnh cầu thận. Do đó, giai đoạn đầu của rối loạn chức năng cầu thận không thể được phát hiện chỉ bằng GFR cơ bản. Mặt khác, tăng huyết áp mao mạch cầu thận được công nhận rộng rãi là một trong những yếu tố nguy cơ mạnh mẽ cho sự tiến triển của các bệnh thận. Hiện nay, rất quan trọng để hình dung động học huyết học cầu thận ở mỗi bệnh nhân mắc bệnh thận nhằm phát hiện sớm rối loạn chức năng, cũng như đánh giá các yếu tố nguy cơ. Tại đây, tôi muốn tóm tắt tình trạng hiện tại của cách phát hiện giai đoạn đầu của rối loạn chức năng thận trong thực hành lâm sàng. Tôi thực sự mong đợi rằng những phương pháp mới để đánh giá động học huyết học cầu thận ở người sẽ được phát triển trong tương lai gần.
