Gibbusresektion bei Myelomeningozelepatienten

Die konservative und chirurgische Behandlung der lumbalen Kyphose bei Patienten mit Myelomeningozele (MMC) ist sehr schwierig und stellt eine therapeutische Herausforderung für den orthopädischen Chirurgen dar. Die kyphotische Deformität im Bereich der LWS kommt zwischen 8% und 20% bei diesem Patientengut vor. Die meisten lumbalen Kyphosen bei diesem speziellen Patientenkollektiv sind sehr rigide und überschreiten bereits bei Geburt einen Krümmungswinkel von 80°. Die Möglichkeiten der konservativen Therapie sind sehr eingeschränkt. Das Tragen einer Rumpforthese ist extrem schwierig, wenig effektiv, in den fortgeschrittenen Stadien eher unmöglich. Die Kyphektomie nach der Operationstechnik von Warner und Fackler wird in unserem Hause seit 1994 als ein Standardverfahren bei der Behandlung von Myelomeningozelepatienten mit einer lumbalen Kyphose angewendet. Aufgrund der Komplikationen in Form von Implantatversagen führten wir diese Studie zur Verbesserung des Verständnisses der Komplikationsursache und zur Optimierung der operativen Technik durch. Das Ziel dieser Studie war die Evaluation der Langzeitergebnisse, der technischen Probleme, der Früh- und Spätkomplikationen sowie der komplikationsassoziierten Risikofaktoren.
