Để xác định các yếu tố dự đoán liên quan đến chất lượng cuộc sống liên quan đến sức khỏe (HRQoL) thấp và trầm cảm ở bệnh nhân viêm cột sống dính khớp (AS) với trọng tâm là sự khác biệt giữa các giới. Chúng tôi đã tiến hành một nghiên cứu đoàn hệ cắt ngang. Dữ liệu lâm sàng liên quan đến AS và các yếu tố bối cảnh đã được thu thập. HRQoL và cảm xúc trầm cảm được đánh giá bằng thang đo EuroQol-5 chiều (EQ-5D) và Thang đo Trầm cảm của Trung tâm Nghiên cứu Dịch tễ học (CES-D), tương ứng. Các phân tích hồi quy logistic đa biến phân tầng theo giới đã được thực hiện. Trong số 211 bệnh nhân, có 161 bệnh nhân nam. Bệnh nhân nam có hoạt động bệnh tương tự và tổn thương hình ảnh x-quang cao hơn so với bệnh nhân nữ. Không có sự khác biệt đáng kể nào trong điểm số chỉ số EQ-5D giữa hai giới. Điểm CES-D cao hơn ở bệnh nhân nữ. Mức độ C-reactive protein (CRP) ASDAS cao hơn có liên quan đến HRQoL thấp ở cả nam (Tỷ lệ Odds [OR] 4.25, khoảng tin cậy [CI] 95% 2.42–7.46) và nữ (OR 2.94, 95% CI 1.02–8.48). Có việc làm liên quan đến khả năng giảm đi việc có HRQoL thấp ở nam (OR 0.39, 95% CI 0.16–0.95). Về trầm cảm, mức độ CRP ASDAS cao hơn (OR 1.87, 95% CI 1.03–3.40), hút thuốc hiện tại (OR 2.98, 95% CI 1.09–8.15), và có việc làm (OR 0.17, 95% CI 0.06–0.46) có liên quan đến trầm cảm ở nam. Đối với nữ, sống cùng với bạn đời có liên quan đến trầm cảm (OR 0.08, 95% CI 0.01–0.93). Các bệnh nhân AS có hoạt động bệnh cao có khả năng gặp phải HRQoL thấp. Cả các yếu tố liên quan đến bệnh và các yếu tố bối cảnh đều có liên quan đến trầm cảm, và các yếu tố dự đoán cho thấy một số khác biệt giữa các giới. Sự nhận thức về sự khác biệt giữa các giới trong việc đánh giá toàn diện có thể dẫn chúng ta đến việc quản lý cá nhân hóa tốt hơn cho các bệnh nhân AS.
