Các phát hiện về tác động của tập thể lực (RT) lên sức mạnh cơ bắp, nồng độ IGF-1 trong huyết thanh, và các protein liên kết của nó là mâu thuẫn nhau. Để giải quyết mâu thuẫn này, chúng tôi đã thực hiện một tổng quan hệ thống và phân tích tổng hợp nhằm khảo sát tác động của RT lên sức mạnh cơ bắp, nồng độ IGF-1 trong huyết thanh, và protein liên kết IGF-3 ở người cao niên. Các cơ sở dữ liệu PubMed, CINAHL, Medline, Google Scholar và Scopus cùng danh sách tham khảo của các nghiên cứu được bao gồm đã được tìm kiếm một cách hệ thống để xác định các thử nghiệm ngẫu nhiên có đối chứng (RCTs) so sánh những đối tượng tham gia RT và những người đối chứng cho đến ngày 15 tháng 5 năm 2020. Nghiên cứu này được thực hiện theo hướng dẫn về các mục ưa thích để báo cáo các tổng quan hệ thống và phân tích tổng hợp. Chúng tôi đã xác định và phân tích 11 thử nghiệm đủ điều kiện trong phân tích tổng hợp này. Dữ liệu tổng hợp cho thấy sự gia tăng đáng kể nồng độ IGF-1 (chênh lệch trung bình (MD): 17.34 ng/ml; khoảng tin cậy 95% (CI): 7.23, 27.46) và sức mạnh cơ bắp ở bài tập đẩy chân (SMD: 0.82; 95% CI: 0.30, 1.34) và bài tập đẩy ngực (SMD: 0.82; 95% CI: 0.42, 1.23) sau khi thực hiện RT. Ngược lại, ước tính tổng hợp cho thấy sự gia tăng không đáng kể ở IGFBP-3 (MD: 0.13 ng/ml; 95% CI: − 39.39, 39.65). Phân tích theo nhóm cho thấy sự gia tăng nồng độ IGF-1 trong huyết thanh sau RT là đáng kể chỉ ở phụ nữ (MD: 19.30 ng/ml); hơn nữa, nó đã tăng sau thời gian can thiệp cả > 12 tuần (MD: 21.98 ng/ml) và ≤ 12 tuần (MD: 15.31 ng/ml). RT gắn liền với sức mạnh cơ bắp tăng lên. Hơn nữa, RT có mối tương quan với sự gia tăng nồng độ IGF-1 trong huyết thanh ở phụ nữ và ở những người tham gia huấn luyện cả ≤ 12 tuần và > 12 tuần. Cần có thêm các nghiên cứu để làm sáng tỏ các cơ chế tác động của RT lên IGF-1, IGFBP-3, và sức mạnh cơ bắp.
