Diagnosis of acetabular loosening is sometime easy, sometime very difficult to do. A correct diagnosis is mandatory. The steps to get the diagnosis and the radiographical views helping to determine the exstension of lysis are described. A good classification of extension of loosening is useful to plan surgery correctly. A correct evaluation of bone defect allows to judge the type of bone loss and the best type of reconstruction. CT scan may be useful to detect any possible pelvic discontinuity, and (by angiography) the relationship of the protruted cup with the vascular bundle.
