Các Xu Hướng Hiện Nay Trong Quản Lý Các Khối U Trực Tràng Thấp: Cắt Cạnh Toàn Bộ Từ Hậu Môn

Việc tiếp cận trực tràng thấp là một thách thức phẫu thuật và trong trường hợp có ung thư, kết quả của bệnh nhân phụ thuộc vào chất lượng ca phẫu thuật. Cắt cạnh toàn bộ từ hậu môn (taTME) là một kỹ thuật phẫu thuật với phương pháp tiếp cận kết hợp bụng và đáy chậu. Chúng tôi xem xét tài liệu về khía cạnh kỹ thuật của taTME cũng như so sánh với các kỹ thuật khác. So sánh giữa cắt cạnh toàn bộ qua nội soi và taTME đã được tóm tắt trong một phân tích tổng hợp cho thấy kết quả ung thư học tốt hơn với tỷ lệ tổn thương bìa vòng thấp hơn và độ hoàn chỉnh của mô trực tràng tốt hơn. TaTME là một phương pháp an toàn. Tất cả các bước của can thiệp được mô tả rõ ràng và cần được tuân thủ vì có rất nhiều cạm bẫy tồn tại. Khi so sánh với TME qua nội soi hoặc robot, taTME cho thấy an toàn với kết quả ung thư học tương đương. Những bệnh nhân khó điều trị, nam giới, béo phì, có khung chậu hẹp nên được xem xét cho phương pháp taTME.
