Phân tích chi phí bệnh phổi tắc nghẽn mạn tính (COPD): một tổng quan hệ thống

Tóm tắt Đặt vấn đề Bệnh phổi tắc nghẽn mạn tính (COPD) là một bệnh có thể điều trị với tỷ lệ mắc cao, và tỷ lệ bệnh tật cao liên quan đến chi phí kinh tế xã hội đáng kể. Mục tiêu Thực hiện một tổng quan hệ thống của tài liệu để phân tích các nghiên cứu chi phí chính liên quan đến COPD, nhằm xác định các yếu tố chính ảnh hưởng đến chi phí của bệnh. Phương pháp Các tìm kiếm đã được thực hiện trong các cơ sở dữ liệu PubMed, SCOPUS và Web of Science cho các nghiên cứu chi phí về COPD được xuất bản bằng tiếng Anh, trong khoảng thời gian từ năm 2015 đến 2020. Các từ khóa tìm kiếm là “COPD” HOẶC “bệnh phổi, tắc nghẽn mạn tính”, “chi phí*” HOẶC “chi phí bệnh”, “tác động kinh tế” VÀ “gánh nặng bệnh tật”. Các tiêu chí bao gồm việc xác định chi phí đã báo cáo của bệnh, gánh nặng kinh tế, chi phí chăm sóc y tế hoặc sử dụng tài nguyên cho COPD, phương pháp được sử dụng, nguồn dữ liệu và các biến được nghiên cứu. Kết quả Có 18 tài liệu được phân tích: 17 tài liệu bao gồm chi phí y tế trực tiếp, 6 bao gồm chi phí không liên quan trực tiếp đến y tế, 12 phân tích chi phí gián tiếp và hai báo cáo chi phí vô hình. Hầu hết các nghiên cứu báo cáo dữ liệu cho các nước phát triển và châu Âu, với chi phí trực tiếp là mảng được nghiên cứu nhiều nhất. Các xu hướng được quan sát thấy trong nhiều nghiên cứu về chi phí trực tiếp và chi phí chăm sóc sức khỏe của các nước châu Âu được đo bằng bệnh nhân và năm, trong đó chi phí cao hơn liên quan đến COPD nặng hơn và lịch sử tái phát thường xuyên. Chi phí cao nhất được báo cáo tương ứng với việc nhập viện và điều trị bằng thuốc liên quan. Vai trò của việc mất năng suất và nghỉ hưu sớm trong hồ sơ bệnh nhân COPD cũng được nhấn mạnh như là yếu tố chính sinh ra chi phí gián tiếp của bệnh.
