Ngày càng nhiều người sử dụng cây cần sa khi khoa học hiện đại và lâm sàng khẳng định và mở rộng các ứng dụng y học của nó. Đồng thời, sự lo ngại và phản đối việc sử dụng thuốc qua đường hít khói cũng đã gia tăng, một phần do những hậu quả gây ung thư đã biết của việc hút thuốc lá. Liệu những phản ứng này có hợp lý hay không? Dù về mặt hóa học rất giống nhau, nhưng có những khác biệt cơ bản trong các thuộc tính dược lý giữa khói cần sa và khói thuốc lá. Khói cần sa chứa các cannabinoid trong khi khói thuốc lá chứa nicotine. Dữ liệu khoa học hiện có, nghiên cứu về các đặc tính gây ung thư của việc hít khói và các hậu quả sinh học của nó, gợi ý về những lý do tại sao khói thuốc lá, nhưng không phải khói cần sa, có thể dẫn đến ung thư phổi.
