Die Ausrichtung der Wirbelsäule verändert sich durch Alter und Degeneration. Die Kompensation der resultierenden Deformität verlagert sich von der Wirbelsäule allein auf das Becken und die unteren Extremitäten. Besonders zwischen der Hüfte und der Wirbelsäule besteht eine enge Wechselwirkung mit Ausprägung von Symptomen, die typisch für das eine wie das andere Erfolgsorgan gewertet werden können und als Hip-Spine-Syndrom bezeichnet werden. Die biomechanischen Grundlagen der Kompensation und die Auswirkung auf die Planung von wirbelsäulen- und hüftchirurgischen Eingriffen werden kritisch dargestellt.
