Các chế độ ăn rất ít carbohydrate hoặc chế độ ăn ketogenic đã được sử dụng từ những năm 1920 như một liệu pháp cho bệnh động kinh và trong một số trường hợp có thể hoàn toàn loại bỏ nhu cầu dùng thuốc. Kể từ những năm 1960, chúng đã trở nên nổi tiếng như một trong những phương pháp phổ biến nhất để điều trị béo phì. Những nghiên cứu gần đây trong khoảng một thập kỷ qua đã cung cấp bằng chứng về tiềm năng điều trị của chế độ ăn ketogenic trong nhiều tình trạng bệnh lý, như tiểu đường, hội chứng buồng trứng đa nang, mụn trứng cá, các bệnh thần kinh, ung thư và giảm thiểu các yếu tố rủi ro của bệnh hô hấp và tim mạch. Khả năng rằng việc điều chỉnh lượng thức ăn có thể hữu ích trong việc giảm hoặc loại bỏ các phương pháp điều trị bằng dược phẩm, thường kéo dài suốt đời và có nhiều tác dụng phụ đáng kể, cần được điều tra nghiêm túc. Bài tổng quan này xem xét lại ý nghĩa của ketosis sinh lý trong ánh sáng của các bằng chứng này và xem xét các cơ chế có thể cho các tác động điều trị của chế độ ăn ketogenic đối với các bệnh khác nhau.
