Basilar Migraine in Childhood

SUMMARYThe incidence and different presentations of basilar migraine are described in a series of 132 children (80 boys, 52 girls) presenting with recurring headache considered to be migraine. Basilar migraine occurred in 29 children, and a further 18 had minor symptoms compatible with but not diagnostic of this form. The course was most often benign, with infrequent, fragmentary attacks and little disability. Rarely, attacks were both severe and frequent, but even then the clinical pattern and outcome usually followed a similar course. Only two children (with the youngest ages at onset) have serious disability, with mental slowing and (in one) permanent neurological sequelae. The relationship between basilar migraine and the syndrome of alternating hemiplegia of infancy is discussed. Because of the limited present knowledge, it is suggested that rigid definitions of migraine should be avoided.RÉSUMÉMigraine basilaire de l'enfantLa fréquence et les différentes manifestations de migraine basilaire sont décrites dans une série de 132 enfants (80 garçons; 52 filles) présentant des maux de tête récidivant, étiquetés migraine. La migraine basilaire est apparue chez 29 enfants, tandis que chez 18 autres enfants des symptômes mineurs compatibles avec cette affection sont apparus, sans possibility d'affirmation diagnostique. l'allure de l'affection était le plus souvent bénigne avec attaques rares, fragmentaires, et peu d'impotence. Les crises étaient rarement à la fois sévères et fréquentes mais dans ce cas, les manifestations cliniques et l'allure de l'affection suivaient un schéma identique. Seuls deux enfants (dont celui ayant débuté les crises le plus tôt) ont présenté une impotence grave avec retard du developpement mental et dans un cas sequelles neurologiques définitives. La relation entre migraine basilaire et le syndrome de l'hémiplégie alternée de la première enfance est discutée. Dans l'état actuel de nos connaissances, il faudrait éviter des définitions strictes de la migraine.ZUSAMMENFASSUNGBasilare Migräne im KindesalterAnhand von 132 Kindern (80 Jungen, 52 Mädchen) mit rezidivierenden Kopfschmerzen, die als Migräne angesehen wurden, wird die Häufigkeit und das unterschiedliche Erscheinungsbild der basilaren Migräne beschrieben. Basilare Migräne trat bei 29 Kindern auf, während 18 geringe Symptome hatten, die zwar mit dieser Form vereinbar waren, jedoch nicht als diagnostische Kriterien dafür anzusehen waren. Der Verlauf war meistens gutartig mit seltenen fragmentarischen Anfallen und geringer Unpäßlichkeit. Nur selten waren es schwere Anfälle, die häufig auftraten und selbst dann nahmen klinisches Bild und Ergebnis einen ähnlichen Verlauf. Nur zwei Kinder (mit dem frühesten Beginn) haben starke Unpäßlichkeiten mit geistiger Verlangsamung und (bei einem) mit bleibenden neurologischen Folgeerscheinungen. Es wird über eine Beziehung zwischen basilarer Migräne und dem Syndrom der alternierenden Hemiplegie im Kindesalter diskutiert. Man weist daraufhin, daß bei derzeitiger Kenntnis eine starre Definition der Migräne vermieden werden sollte.RESUMENMigran̄a basilar en la infanciaSe describe la incidencia y diferentes presentaciones de la migrana basilar en una serie de 132 nin̄os (80 muchachos, 52 chicas) que aparece con cefalalgia recurrente considerada como migran̄a. La migran̄a basilar ocurrió en los 29 nin̄os, mientrasque 18 tenían síntomas menores compatibles con, pero no diagnosticados de, esta forma. El curso por regla general era benigno, con ataques infrecuentes fragmentarios de pequen̄a incapacidad. Raramente los ataques eran al mismo tiempo graves y frecuentes, incluso entonces el esquema clfnico y el curso posterior generalmente seguían un mismo esquema. Solamente (dos nin̄os) con la edad más joven en el inicio, tenían minusvalencia seria con enlentecimiento mental y (en un caso) secuelas neurológicas permanentes. La relación entre la migran̄a basilar y el síndrome de la hemiplejia alternante de la infancia es objeto de discusión. Se sugiere que con unicamente el conocimiento actual, las definiciones rígidas de migran̄a debenían ser evitadas.
