Mối quan hệ giữa các đặc điểm sinh sản ở phụ nữ và các sự kiện đột quỵ vẫn chưa rõ ràng. Chúng tôi đã tiến hành nghiên cứu về tuổi bắt đầu hành kinh, tuổi mãn kinh và số năm sinh sản liên quan đến tỷ lệ đột quỵ fatal trong Nghiên cứu Biobank Quảng Châu. Tổng cộng có 16.504 phụ nữ mãn kinh không bị đột quỵ, bệnh tim hoặc lịch sử ung thư tại thời điểm ban đầu đã được đưa vào và theo dõi trong thời gian trung bình 12.0 năm. Sau khi kiểm tra những hồ sơ có sẵn, có 222 trường hợp tử vong do đột quỵ được ghi nhận. Phân tích hồi quy nguy cơ tỷ lệ Cox đã được sử dụng để đánh giá các mối liên hệ giữa nguy cơ xảy ra đột quỵ fatal và tuổi bắt đầu hành kinh, tuổi mãn kinh và số năm sinh sản. Trong toàn bộ nhóm nghiên cứu, so với những người có tuổi bắt đầu hành kinh là 15 tuổi, những phụ nữ có tuổi bắt đầu hành kinh là 12 tuổi có nguy cơ đột quỵ fatal cao hơn với aHR (tỷ lệ nguy cơ điều chỉnh) là 1.86 (Khoảng tin cậy 95% (CI) 0.96–3.60), 13 tuổi (aHR = 1.69, 95% CI 0.98–2.92), 17 tuổi (aHR = 1.83, 95% CI 1.10–3.05) và ≥ 18 tuổi (aHR = 1.66, 95% CI 1.03–2.70), trong đó các mối liên hệ cho thấy hình dạng U không điển hình; mối liên hệ hình U tương tự ở nhóm phụ nữ mãn kinh sinh trước năm 1940 đã cho thấy một loạt các nguy cơ gia tăng của đột quỵ fatal ở phụ nữ có tuổi bắt đầu hành kinh khi ≤ 12 tuổi (aHR = 3.68, 95% CI 1.68–8.05), 13 tuổi (aHR = 2.11, 95% CI 1.02–4.34), 14 tuổi (aHR = 2.07, 95% CI 1.04), 17 tuổi (aHR = 2.30, 95% CI 1.20–4.39) và 18 tuổi (aHR = 2.50, 95% CI 1.37–4.57), tương ứng. So với các phụ nữ mãn kinh trong độ tuổi 51–52, những người < 43 tuổi at mãn kinh có nguy cơ gia tăng đột quỵ fatal ở nhóm phụ nữ mãn kinh sinh ra từ năm 1940 trở đi (aHR = 1.64, 95% CI 0.97–2.78) và phụ nữ mãn kinh sinh trước năm 1940 (aHR = 1.97, 95% CI 1.05–3.69). Thêm vào đó, so với những người có 32–34 năm sinh sản, phụ nữ có ≤ 28 năm sinh sản có nguy cơ gia tăng cho đột quỵ fatal trong toàn bộ nhóm nghiên cứu (aHR = 1.91, 95% CI 1.28–2.86) và nhóm phụ nữ mãn kinh sinh trước năm 1940 (aHR = 1.79, 95% CI 1.15–2.80). Tuổi bắt đầu hành kinh trẻ hơn và cao hơn, tuổi mãn kinh trẻ hơn và số năm sinh sản ít hơn có liên quan đến nguy cơ gia tăng đột quỵ fatal ở phụ nữ mãn kinh.
