Với sự gia tăng toàn cầu trong việc sử dụng chất làm đầy tiêm, ngày càng nhiều trường hợp có tác dụng phụ nghiêm trọng như mất thị lực đang được báo cáo. Bài viết này nhằm xem xét các trường hợp tổn thương thị giác liên quan đến chất làm đầy axit hyaluronic (HA) và thảo luận về hiệu quả tiềm năng của việc điều trị bằng hyaluronidase (HYASE) thông qua các phương pháp tiêm khác nhau. Tổng cộng có 29 bài báo trình bày 144 trường hợp tổn thương thị giác liên quan đến chất làm đầy HA được đưa vào nghiên cứu này. Phần lớn các trường hợp tổn thương thị giác liên quan đến chất làm đầy HA được báo cáo từ Trung Quốc, tiếp theo là Hàn Quốc. Đối tượng chủ yếu là phụ nữ. Mũi, trán và gò má là các vị trí tiêm phổ biến nhất. Tất cả các trường hợp đều có sự suy giảm thị lực và gần như tất cả đều là tổn thương một bên với sự xuất hiện ngay lập tức các dấu hiệu và triệu chứng thị giác. Tắc động mạch mắt (OAO) và tắc động mạch võng mạc trung tâm (CRAO) là hai mẫu tắc mạch phổ biến nhất với tiên lượng rất kém, tiếp theo là tắc động mạch võng mạc nhánh (BRAO), đây là mẫu động mạch có tiên lượng tốt hơn. HYASE tiêm dưới da và vào động mạch đều hỗ trợ phục hồi thị lực ở các mức độ khác nhau, trong khi HYASE tiêm qua vùng quanh nhãn cầu dường như ít hữu ích hơn. Các biến chứng sau khi tiêm chất làm đầy dựa trên HA rất hiếm, nhưng có thể gây tổn hại thị giác nghiêm trọng. HYASE tiêm dưới da và vào động mạch đều giúp phục hồi thị lực ở những mức độ khác nhau, và hiệu quả có thể được tăng cường khi thực hiện cùng với nhau, trong khi HYASE tiêm qua vùng quanh nhãn cầu dường như ít hữu ích hơn. Tuy nhiên, để đánh giá chính xác hiệu quả của HYASE thông qua các phương pháp sử dụng khác nhau, cần tiến hành thêm các thử nghiệm lâm sàng ngẫu nhiên có đối chứng.
