Tóm tắt Nivolumab là một kháng thể chống chết tế bào theo chương trình-1 (PD-1) được sử dụng như một chất ức chế điểm kiểm soát miễn dịch cho một số loại ác tính. Tuy nhiên, tác nhân này có liên quan đến các sự kiện bất lợi liên quan đến miễn dịch (irAEs), chủ yếu trong phổ bệnh tự miễn, bao gồm viêm phổi kẽ, viêm đại tràng, tiểu đường týp 1 và suy thận. Chúng tôi báo cáo trường hợp của một nam bệnh nhân 59 tuổi mắc ung thư tế bào thận, người đã phát triển tình trạng chức năng thận xấu đi khoảng 4 tháng sau khi bắt đầu điều trị với nivolumab. Phân tích nước tiểu cho thấy protein niệu và tiểu máu vi thể cùng với sự gia tăng nồng độ N-acetyl-β-D-glucosaminidase. Sinh thiết thận cho thấy viêm thận ống mô kẽ cấp tính và sự dày lên của màng đáy cầu thận. Kỹ thuật miễn dịch huỳnh quang cho thấy có lắng đọng IgM dạng hạt trong các vòng mao mạch. Chúng tôi đã bắt đầu liệu pháp với prednisolone liều cao kết hợp với nivolumab, điều này đã cải thiện chức năng thận và đạt được sự thuyên giảm hoàn toàn protein niệu. Mặc dù các irAEs liên quan đến thận được coi là hiếm và viêm cầu thận không phải là biểu hiện điển hình, các bác sĩ cần theo dõi chặt chẽ chức năng thận và phân tích nước tiểu ở những bệnh nhân đang điều trị miễn dịch với các tác nhân này. Ngoài ra, trường hợp của chúng tôi cung cấp một liên kết khả dĩ giữa nivolumab và viêm cầu thận do miễn dịch.
