A new surgical treatment of acne conglobata (bucket-handle flap)

We present the case of a 35-year-old man suffering from severe acne conglobata with significant discharge causing social discomfort and problems for 15 years who was treated with a new surgical technique. There were no complications or recurrence during a 2-year follow-up period. The advantages of this surgical technique are as follows: it does not require a secondary donor site; the flaps have good vascular supply; and it is easy and less time consuming to perform when there is good surgical exposure. The bipedicled random pattern flap (bucket-handle flap) is an alternative surgical technique for cases of acne conglobata which are resistant to medical treatment especially in the gluteal region.
