Arthroskopische Stapling-Operation am Schultergelenk bei vorderer Instabilität

Endoskopische Refixation oder Straffung des Limbus-Ligg.-glenohumeralia-Komplexes, dessen Insuffizienz nach den Untersuchungen von Turkel et al. [9] bei der Entstehung anteroinferiorer instabilitäten entscheidende Bedeutung zukommt. In der Regel ist der Limbus glenoidalis an Skapulahals und Pfanne unter Verlust seiner Bandansatzfunktion abgerissen. Zu beheben ist also die sogenannte Bankart-Läsion, selten (höchstens in 10%) die reine substantielle Dehnungsverletzung und Vernarbung der Ligg. glenohumeralia. Auch sonst wird ihre mäßige Straffung angestrebt („Capsular Shift”). Am weitesten verbreitet ist die Limbusrefixation mit Staples (Alternativen siehe unten!).
