Kollumrekonstruktion nach Resektion einer intrakapsulären Magenkarzinommetastase

Die Mandibula stellt eine sehr seltene Lokalisation für Magenkarzinommetastasen dar. Normalerweise bleibt der hohe kondyläre Bereich unberührt. Als Grund wird u. a. die eigenständige Blutversorgung diskutiert. Problematisch ist die Rekonstruktion nach einer Resektion, da häufig eine frühzeitige adjuvante systemische Therapie erforderlich ist. Am Beispiel dieser Metastasenresektion wird der Einsatz einer Quinn-Kiefergelenkprothese vorgestellt. Wir berichten über ein hämatogen metastasierendes Magenadenokarzinom eines 51-jährigen Patienten, bei dem sich initial eine zunehmende Disklusion im Seitzahnbereich zeigte. Wegen des Verdachts auf eine intraartikuläre Fernmetastase erfolgte eine Kondylektomie mit Sofortrekonstruktion mittels totaler Kiefergelenkpfannenprothese über einen präaurikulären Zugang. Die klinische und bildgebende Verdachtsdiagnose wurde histologisch nach Entkalkung als intrakapsuläre Adenokarzinommetastase bestätigt. Nach Resektion dieser nach unseren Recherchen weltweit ersten dokumentierten intrakapsulären Metastase eines Magenkarzinoms verlief die Einheilungsphase dieser modifizierten Quinn-Prothese schnell und komplikationslos. Bei frühzeitiger Wiederherstellung der Funktion und Beschwerdefreiheit konnte die anstehende radiochemotherapeutische Behandlung weiterer, nicht resezierbarer Fernmetastasen zeitgerecht eingeleitet werden. Am Beispiel dieses extrem seltenen Metastasenfalles konnte gezeigt werden, dass eine solche Gelenkprothese eine sehr gute Alternative zu einer aufwendigen autologen Rekonstruktion oder einer unbefriedigenden Girdlestone-Situation darstellt. Insbesondere die modifizierte Quinn-Prothese erweist sich aufgrund des Oberflächendesigns des sphärischen Kondylus und der Gelenkpfanne als besonders geeigneter totaler Gelenkersatz nach ausgedehnter Resektion oder auch multiplen Voroperationen.
