Die ischämische Mitralinsuffizienz ist eine häufige und prognostisch ungünstige Komplikation der koronaren Herzkrankheit. Sie wird verursacht durch Myokardischämie und/oder strukturelle Veränderungen des linken Ventrikels nach Myokardinfarkt, die über Verlagerung der Papillarmuskeln zur Behinderung des Klappenverschlusses führen. Die ischämische Mitralinsuffizienz ist abhängig von Vorlast und Nachlast, deshalb nimmt ihr Schweregrad unter Anästhesie ab. Perioperativ können verschiedene Faktoren eine neue oder eine Verschlechterung einer vorbestehenden ischämischen Mitralinsuffizienz verursachen. Für Diagnose, Quantifizierung sowie Steuerung der Therapie der perioperativen ischämischen Mitralinsuffizienz ist die Echokardiographie, insbesondere die transösophageale Echokardiographie, ausschlaggebend
