Les troubles des conduites alimentaires peuvent être considérés comme un passage à l’acte qui court-circuite la représentation et la mentalisation. Ce passage à l’acte a ceci de particulier qu’il vise le corps propre et non l’environnement comme dans les formes psychopathiques classiques. Dans sa genèse, le processus est clairement lié au style de la relation mère-enfant au cours des premiers mois. Quel que soit le type de mère en cause, il apparaît indispensable, pour qu’une bonne individuation se constitue, que le temps du manque, prélude au désir, puisse se mettre en place. C’est ainsi que la faim pourra être différenciée des autres besoins ou affects. Ces notions apparaissent essentielles pour que le thérapeute puisse, au cours du travail psychologique, trouver « la bonne distance » et être une assez bonne mère pour son patient.
