Die komplette herzerhaltende Palliation beinhaltet neben der Norwood-Operation im Neugeborenenalter auch die spätere Kreislauftrennung nach Fontan. Unsere Erfahrungen mit den einzelnen operativen Teilschritten werden vorgestellt. 43 Säuglinge mit anatomischem (n = 33) oder funktionellem (n = 10) HLHS erhielten von 1989 bis 1996 eine Norwood-Operation. Funktionelle HLHS waren: Mitralatresie mit doppeltem Ausgang aus der rechten Kammer und subaortaler Stenose (n = 2), atrioventrikulärer Septumdefekt mit hypoplastischer linker Kammer, Subaortenstenose und Aortenkoarktation (n = 1), hypoplastischer, subaortaler rechter Ventrikel mit restriktivem Ventrikelseptumdefekt und Aortenhypoplasie (n = 7). Das mediane Operationsalter betrug 15 Tage (5 bis 182 Tage), das mittlere Gewicht war 3,3 kg (3,0 bis 4,9 kg). Die operative Sterblichkeit lag bei 32% (n = 14), wobei sie seit 1994 auf 16% (3 von 19 Patienten) gesenkt werden konnte. 5 Patienten (12%) verstarben 2 Wochen bis 6 Monate später, 2 Kinder wurden herztransplantiert. Von den 22 lang Überlebenden erhielten bisher 19 Patienten im medianen Alter von 7 Monaten (2 bis 14 Monate) die obere, bidirektionale kavopulmonale Anastomose als Hemi-Fontan. 2 Kinder verstarben (10%).Von den 17 Überlebenden erhielten bisher 12 nach einem mittleren Intervall von 12 Monaten die ergänzende kavopulmonale Anastomose. Alle Kinder überlebten den Eingriff und sind damit fertig palliiert. Die längste Nachbeobachtung nach kompletter Fontan-Operation liegt bisher bei 6 Jahren. Zusammenfassung: Die Ergebnisse der Norwood-Operation werden mit wachsender Erfahrung günstiger. Ergebnisse: Frauen waren im Durchschnitt älter, hatten eine bedeutsamere Komorbidität und hatten eine längere Prähospitalphase als Männer. Die PTCA des Infarktgefäßes war erfolgreich bei 95% der Frauen und 94% der Männer. Die kumulative Gesamtmortalität während der ersten 4 Jahre war 12,5%, 14,5% 18% und 23% bei Frauen und 9%, 10,5%, 12% und 15% bei Männern (p=ns bis Jahr 3, p\u003C0,05 ab dem 4. Jahr). Recently changes in vascular geometry in the form of positive (compensatory enlargement) as well as negative remodeling (constrictive changes) could be demonstrated in transplanted hearts as well. For further characterization intravascular ultrasound provides an excellent insight into time-related changes (by serial follow-up examinations) and into longitudinal spatial relationships within vessels (motorized pullback with 3-D reconstruction). Focused on time-related changes a decrease in total vessel volume within the first year after transplantation as compared to a slight compensatory enlargement in patients ‘late’ after HTX could be demonstrated. Regarding focal luminal obstruction, local constrictive changes (negative remodeling) were identified to be the most important predictor of severity of stenosis. It is concluded that changes in vascular geometry are of major importance in focal stenosis as well as in a general luminal loss of coronary artery within transplanted human hearts. Die invasive Diagnostik ergab den Befund einer koronaren Drei-Gefäßerkrankung mit subtotaler Stenose der RCA und des linken Hauptstammes. Aufgrund der Hauptstammstenose wurde die Indikation zur operativen Revaskularisierung gestellt, ein Venenbypass auf die RCA angelegt und eine Patchplastik des Hauptstammes durchgeführt.   Bei postoperativ komplikationslosem Verlauf wurde aufgrund der vitalen Bedeutung eines großlumigen Hauptstammes früh-postoperativ eine Kontroll-Koronarangiographie mit intravaskulärem Ultraschall (IVUS) des Hauptstammes durchgeführt.   Es werden die historische Entwicklung, Indikationen und die wesentlichen Vorteile der Patch-Plastik gegenüber der konventionellen Bypass-Operation diskutiert. Der Schwerpunkt wird auf den Stellenwert des IVUS in der prä- und postoperativen Beurteilung des linken Hauptstammes gelegt.   Der intravaskuläre Ultraschall hat sich in den letzten Jahren zum Goldstandard der bildgebenden Diagnostik der Koronargefäße entwickelt. Eine zunehmend wichtigere Rolle spielt er bei der Evaluierung angiographisch nicht sicher beurteilbarer Läsionen im linken Hauptstamm und der Bifurkation des Hauptstammes. Darüber hinaus wurde berichtet, daß das Risiko von Embolie-Rezidiven bei diesen Patienten 3–4% pro Jahr beträgt. Eine zuverlässige Risikostratifikation der Patienten mit PFO auf der Basis klinischer oder echokardiographischer Befunde ist bisher – mit Ausnahme der hämodynamisch wirksamen Lungenembolie – in der Regel nicht möglich. Das Vorliegen eines Vorhofseptumaneurysmas und eine weite Öffnung des PFO während des Herzzyklus oder ein großer interatrialer Shunt bei der transösophagealen Kontrast-Echokardiographie werden jedoch von manchen Autoren als Risikofaktoren angesehen. Die sekundäre Prävention paradoxer Embolien mittels oraler Antikoagulanzen (Warfarin) beinhaltet das Risiko einer signifikanten Blutung (Inzidenzrate 2–5% pro Jahr) und mangelhaften Compliance der Patienten. Außerdem scheint diese Therapieform wenig effektiv zu sein. Der chirurgische Verschluß des PFO ist ein technisch ausgereifter, sicherer Eingriff. Erste Berichte bestätigen seine hohe Effektivität hinsichtlich der Rezidivprophylaxe, weitere prospektive Studien sind jedoch erforderlich, um die für eine chirurgische Behandlung geeigneten Patienten festzulegen. Kathetertechnische Verfahren zum PFO-Verschluß stehen ebenfalls zur Verfügung und befinden sich derzeit in der Phase der klinischen Erprobung. Bisher sind noch nicht alle Faktoren geklärt, welche die Restenoserate beeinflussen. Daher wurde in der vorliegenden Studie der Einfluß zweier unterschiedlicher Stentdesigns, der Implantationsart (Hochdruckimplantation > 10 atm, Niederdruckimplantation ≥ 10 atm), der Lokalisation der Stenose sowie der Stenose-Morphologie untersucht. Hierzu wurden die koronarangiographischen Untersuchungen von 259 Patienten, bei denen in 307 Stenosen insgesamt 196 Micro- und 142 Palmaz-Schatz-Stents implantiert worden waren, quantitativ vor, direkt nach und 3-6 Monate nach Stent-Implantation retrospektiv ausgewertet. Es fand sich zwischen den beiden Stent-Typen kein signifikanter Unterschied in der Restenoserate (Palmaz-Schatz 33%, Micro 27%). Bei Hochdruckimplantation des Micro-Stents mit Drucken von > 10 atm zeigte sich jedoch eine signifikant niedrigere Restenoserate sowohl im Vergleich zu den mit niedrigen Drucken implantierten Micro-Stents (17% versus 35%, p \u003C 0,02) als auch im Vergleich zu den nahezu ausnahmslos mit Hochdruck implantierten Palmaz-Schatz-Stents (17% versus 34%, p \u003C 0,02). Lediglich 11 Palmaz-Schatz-Stents wurden mit einem Druck von ≥ 10 atm implantiert. Ein Grund hierfür war aus den Untersuchungsprotokollen retrospektiv nicht zu entnehmen. In der LAD lag die Restenoserate mit 39% signifikant höher als in der RCX (21%) und RCA (22%, p \u003C 0,02). Bei komplexen Typ-C-Stenosen zeigt sich eine höhere Restenoserate (39%) als bei Typ-A- (17%) und Typ-B-Stenosen (26%, p \u003C 0,05).
