partial nephrectomies were performed without renal cooling on 13 pigs with a contact fibre Nd: YAG laser (10 W) or a steel scalpel with or without a vascular pedicle clamp. Nine pigs had a 2-week follow-up with an abdominal ultrasound 1 week postoperatively. The time for haemostasis was 6.9±5.8 min (mean ±SD) with the laser and 9.1±5.8 min with the steel scalpel when the clamp was used (P=0.028). There was no significant difference in the total operating time (13.2±4.5 min with the laser vs 12.6±4.6 min with the steel scalpel, P=0.203). Intraoperative blood loss was similar in the two groups when the clamp was used. Clamping the renal pedicle decreased the blood loss by 61% in the laser group and 31% in the steel scalpel group. The number of ligatures used was significantly lower in the laser group (3.7±2.6) compared with the steel scalpel group (6.6±3.4) (P=0.013). Five urinomas developed on the laser side and four on the steel scalpel side. These results indicate that the contact fibre Nd:YAG laser method can be used in partial nephrectomy, but it offers no definitive advantage over the conventional technique.
Contact fibre Nd: YAG laser for partial nephrectomy: experimental study in pigs
Barzilay B, Lijovetzky G, Perlberg S, Caine M (1982) Comparative experimental study on the use of the carbon dioxide laser beam in partial nephrectomy. investigate the cytotoxic activity of 7-N-(2-([2-(gamma-l-glutamylamino)ethyl]dithio)ethyl)-mitomycin C (KW-2149) versus mitomycin C (MMC) against cell lines from human transitional cell carcinoma (TCC). Direct cytotoxicity of the two drugs was measured employing a colorimetric cytotoxicity assay on chemonaive and chemoresistant cancer cell populations. The results revealed that all cell lines (n= 19) were significantly more inhibited by treatment (2 h, 96 h) with KW-2149 than by MMC (P < 0.03–0.001). pH 6.0 decreased the stronger activity of KW-2149 (P < 0.013–0.004). Creatinine ≥10 mmol/l and nitrosourea ≥100 mg/l also inhibited the activity of KW-2149 significantly. Tumor cells with relative drug-resistance against MMC (RT112-MMC: 55-fold) exerted minor cross-resistance to KW-2149 (fourfold). In conclusion, the present in vitro data suggest KW-2149 to be a superior drug for intravesical therapy of patients with primary or recurrent superficial bladder carcinoma. Since pH and concentrations of creatinine and nitrosourea influence the activity of KW-2149, patients are supposed to profit from neutralizing the urinary pH and enhanced diureses.","http://link.springer.com/10.1007/s002400050052",[1200,1215,1228,1241,1254
"9158bac9-58f7-4ffa-bff3-2f5ba66c3c99",[974],[1204
"2627d467-07b9-4cb4-b048-a9c529201f90
[
Department large series of transitional cell tumours has been screened for the presence of human papillomavirus (HPV) sequences using Southern blotting and general primer-mediated polymerase chain reaction (GP-PCR). The latter technique allows the detection of a broad spectrum of both sequenced and unsequenced HPV types using two primer pairs located in the highly conserved L1 and E1 regions of the HPV genome. No evidence for HPV infection was found in 100 transitional cell tumours, 6 cases of carcinoma in situ, 2 adenocarcinomas and a squamous carcinoma of the bladder and 3 cases of cystitis. Similarly, 12 bladder tumour cell lines were HPV-negative in these assays. Cervical carcinoma cell lines containing from 1–3 to 600 copies of the HPV genome were used as positive controls and were scored positive in all assays by both Southern blotting and GP-PCR. It is concluded that despite the close proximity of the urothelium to the genital mucosa and the resemblance of some bladder tumours to known HPV-induced lesions in other tissues, HPV infection is absent or very uncommon in bladder tumours.
Human papillomavirus sequences are not detectable by Southern blotting or general primer-mediated polymerase chain reaction in transitional cell tumours of the bladder
Auersperg N (1964) Long-term cultivation of hypodiploid human-tumor cells. It was first demonstrated by Young [171, 172] that spermatozoa undergo certain changes as they migrate through the epididymis. He proposed that spermatozoa ripen during epididymal transit. It is now known that specific maturational changes occur in spermatozoa during epididymal transit which result in their developing the ability to fertilize ova. Concomitant with this functional maturity are changes in spermatozoal morphology, motility, chemistry, permeability, density and metabolism. It is apparent that in some way not understood these changes are necessary for sperm to achieve the ability to complete the fertilization process. When these mechanisms are understood, we may be able to effectively treat conditions such as necrospermia or abnormally low sperm motility. Furthermore, with the development of the hamster-egg penetration test a “new” type of male infertility has become evident in recent years; the inability of otherwise normal sperm to penetrate an ovum. It is during epididymal transit that this ability is normally acquired. Thus, any insight into how sperm attain the capacity to penetrate an ovum could lead to an effective treatment of patients whose sperm do not have this ability. In addition, the epididymis holds significant promise as the site of action for a male contraceptive. Thus, it is the purpose of this review to describe the structure and function of the mammalian epididymis with particular emphasis on the factors regulating sperm maturation.
Review article: Structure and function of the epididymis
Acott TS, Hoskins DD (1978) Bovine sperm forward motility protein. The value of photodynamic diagnosis (PDD) and autofluorescence imaging (AF) in detecting malignant lesions on the penis was evaluated. Twelve patients with biopsy-confirmed squamous cell cancer (SCC) of the penis were examined with PDD and AF. For the PDD and AF the penis was illuminated with the blue excitation light from a xenon arch lamp. Biopsies were taken from suspicious lesions detected by PDD or AF and then treated with Nd:YAG laser coagulation. Neoplastic lesions presented with a positive red fluorescence under PDD or a diminished appearance under AF. The HPV-analysis was positive in eight of the 12 lesions. Fluorescence diagnosis is used for the detection of neoplastic lesions. It assists the urologist in detecting neoplastic and preneoplastic lesions, ensuring a more reliable destruction of all tumor material in penile sparing Forty-six infants with renal stones sizing 15–30 mm were treated by either ESWL (22 renal units in 22 infants) using Dornier compact delta lithotripter or MPCNL (25 renal units in 24 infants) using 14F–18F renal access under general anesthesia. The operation time, stone-free rate, re-treatment rate, and complications between the two groups were compared with the χ2, Mann–Whitney U, and Student’s t tests. No significant differences in mean age and stone size were observed between the two groups. The 1- and 3-month postoperative stone-free rates were 84 and 96 % in MPCNL group and were 31.8 and 86.4 % in ESWL group. The re-treatment and complication rates were significantly higher in ESWL group than in MPCNL group (50 vs. 12 %, P = 0.004; 16.0 vs. 45.5 %, P = 0.028). The stone recurrence rate was similar between the two groups. No significant changes of serum creatinine (Cr) level and glomerular filtration rate were observed in both groups. In conclusion, MPCNL is an effective and feasible alternative monotherapy for large renal stones (15–25 mm) in infants, with a higher stone-free rate and a lower complication rate when compared with ESWL.
Treatment of renal stones in infants: comparing extracorporeal shock wave lithotripsy and mini-percutaneous nephrolithotomy
citation_journal_title=J Urol; citation_title=Extracorporeal shock wave lithotripsy as first line treatment alternative for urinary tract stones in children: a large scale retrospective analysis; citation_author=AY Muslumanoglu, A Tefekli, O Sarilar; citation_volume=170; citation_publication_date=2003; citation_pages=2405-2408; citation_doi=10.1097/01.ju.0000096422.72846.80; citation_id=CR1
citation_journal_title=J Endourol; citation_title=Pediatric extracorporeal shockwave lithotripsy: its efficiency at various locations in the upper tract; citation_author=FT Hammad, M Kaya, E Kazim; citation_volume=23; citation_publication_date=2009; citation_pages=229-235; citation_doi=10.1089/end.2008.0133; citation_id=CR2
citation_journal_title=Urology; citation_title=Extracorporeal shock wave lithotripsy monotherapy for renal stones >25 mm in children; citation_author=AM Shouman, AM Ziada, IA Ghoneim; citation_volume=74; citation_publication_date=2009; citation_pages=109; citation_doi=10.1016/j.urology.2008.09.083; citation_id=CR3
citation_journal_title=Urol Res; citation_title=Shockwave lithotripsy and endourological stone treatment in children; citation_author=S Lahme; citation_volume=34; citation_publication_date=2006; citation_pages=112-117; citation_doi=10.1007/s00240-005-0021-5; citation_id=CR4
citation_journal_title=J Endourol; citation_title=Results of shockwave lithotripsy for pediatric urolithiasis; citation_author=AH Tan, M Al-Omar, JD Watterson; citation_volume=18; citation_publication_date=2004; citation_pages=527-530; citation_doi=10.1089/end.2004.18.527; citation_id=CR5
citation_journal_title=J Urol; citation_title=Management of pediatric urolithiasis in Pakistan.
