kidney disease (CKD) and anemia are independent preoperative risk factors for coronary artery bypass graft (CABG) surgery. We evaluated the implications of the coexistence of these two factors and their associated prognosis for CABG surgery. We analyzed, retrospectively, consecutive patients who underwent elective CABG surgery between 2004 and 2014. The patients were classified into four groups depending on the presence or absence of preoperative CKD and anemia. We assessed the major adverse cardiac and cerebrovascular event (MACCE), defined as composite outcomes of cardio- and cerebrovascular death, revascularization through surgery or percutaneous intervention, hospitalization for congestive heart failure, and cerebral infarction. The study population consisted of 510 patients (73 % male; median age 71 years old), followed up for a median period of 2.8 years. Multivariate analysis indicated that neither the CKD\u002Fno-anemia group [hazard ratio (HR) 0.98, 95 % confidence interval (CI) 0.39–2.51, P = 0.973] nor the no-CKD\u002Fanemia group (HR 1.20, 95 % CI 0.69–2.09, P = 0.512) had significantly poorer prognoses than the no-CKD\u002Fno-anemia group. chemo-immunotherapy program was designed on the basis of our clinical findings, which revealed that the ability to generate cell-mediated cytotoxicity was remarkably augmented after adriamycin (AM) administration in cancer patients. Twenty patients with stage IV gastric carcinoma who had undergone noncurative resections were treated with the above regimen that consisted of active immunotherapy with autologous tumor cells, in combination with 30 mg of AM, given 7 days before the immunization, followed by long-term tegafur (FT) and immunomodulators. The survival of these patients was compared to that of 3 other groups of patients, namely; 30 patients treated with another chemo-immunotherapy regimen which comprised autologous tumor cells in combination with several anticancer drugs followed by long-term FT and immunomodulators (CCI1), 17 patients treated with mitomycin C followed by long-term FT and immunomodulators (CI), and 24 patients with comparable histories (HC). With regard to the regional wall motion, the impaired LAD-ΔL (the segment systolic shortening) and LAD-Elmax (the slope of peak systolic pressure—endsystolic length relation) following the LAD occlusion improved from 0.5±2.5 per cent to 5.9±2.6 per cent (p\u003C0.01) and from 50±9 to 82±14 mmHg\u002Fmm (p\u003C0.01) after the infusion of dopamine with bunazosin. Dopamine greatly increased the Rate Pressure Product (RPP) from 12610±1120 after LAD occlusion to 16950±1420, whereas dopamine in combination with bunazosin did not increase the RPP due to a drop of LV-PSP with little change in regional myocardial blood flow. This study evaluated the effectiveness of surgical suturing training as an educational tool using the Leap Motion Controller (LMC), which can capture hand movements and reproduce them as data comprising parametric elements. We developed an off-the-job training system (Off-JT) in our department, mainly using prosthetic grafts and various anastomotic methodologies with graded difficulty levels. We recruited 50 medical students (novice group) and 6 vascular surgeons (expert group) for the study. We evaluated four parameters for intraoperative skills: suturing time, slope of the roll, smoothness, and rate of excess motion. All 4 parameters distinguished the skill of the novice group at 1 and 10 h off-JT. After 10 h of off-JT, all 4 parameters of the novices were comparable to those of the expert group. hepatopulmonary syndrome (HPS) is a rare complication of decompensated liver cirrhosis and a relative contraindication for liver transplantation. This report describes the treatment of two cases with severe HPS resulting from chronic liver failure by living donor liver transplantation (LDLT). The clinical data of the two cases were reviewed. The parameters included the intrapulmonary shunt ratio as measured by 99mTc pulmonary scintigraphy, perioperative treatment and examination results, and the duration of post-transplant hypoxemia. Liver dysfunction was reversed in both patients 2–3 weeks after LDLT. Both patients experienced slightly increased intrapulmonary shunt ratios in the first postoperative month, followed by substantial decreases at postoperative days 90 and 361. These findings suggest severe HPS can be resolved by LDLT. Pulmonary infection requires proper treatment and may be anticipated during the early postoperative course. report a case of typical fibrolamellar hepatocellular carcinoma (FL-HCC) in a 16-year-old Japanese boy. This is very rare malignancy in Japan. The patient was referred for investigation of a large hepatic tumor and the results of tests for hepatitis B virus surface antigen and hepatitis C virus antibody were negative. Liver function test results and serum alpha-fetoprotein (AFP) levels were normal; however, the prothrombin induced by vitamin K absence\u002Fantagonist II was elevated. Computed tomography (CT) showed a large lobulated heterogeneously enhanced tumor in the posterior section of the liver. We diagnosed FL-HCC and performed posterior sectionectomy of the liver. The resected specimen contained a light brown and green tumor with a central fibrous scar, 10.0 cm in diameter. Microscopic and electron microscopic examinations revealed the typical features of FL-HCC. Immunohistochemically, the tumor cells were positive for cytokeratin (CK) 7, but negative for CK19 and AFP. The patient was alive without recurrence 48 months after surgery. evaluated the long-term results in 43 patients who had undergone open mitral commissurotomy for recurrent mitral stenosis after closed commissurotomy. There was one operative death (2.3 percent) and one case of reoperation (2.3 percent). Twenty patients (42 percent) were in NYHA Class I and 22 patients (51 percent) in Class II. In 12 randomly selected patients subjected to postoperative cardiac catheterization, the pulmonary arterial mean pressure remained high at rest and increased to 48 mmHg (mean) during exercise with significant differences in patients who underwent open mitral commissurotomy in the first operation (control group). The left atrial mean pressure was significantly high at rest and rose to 27 mmHg (mean) during exercise. The pulmonary vascular resistance also increased during exercise, with significant differences for the control group.
