The hearing-impaired children are known to have a pragmatic language delay as hearing impairment deprived of exposure to natural communication interactions, in addition to the language delay they have. Since the age of implantation has emerged as an important predictor of language, hearing, and speech in children who use cochlear implants (CI), question aroused about the benefits of early cochlear implantation on pragmatic language development in those children. Thus, this study aims to compare the pragmatic language development of the prelingual hearing impaired children who cochlear implanted before the age of 3 years and those who cochlear implanted after the age of 3 years. The two study groups showed no significant differences regard their scores in the Egyptian Arabic Pragmatic Language Test (EAPLT). The two studied groups had pragmatic language scores below their 5th percentile. Among the studied groups, the scores of the EAPLT were positively correlated to the age of the children, the children’s language abilities, and the duration of the received language rehabilitation, with no significant correlation to the age of implantation. The age of implantation has no impact on pragmatic language development in children with CI. The prelingual children with CI are susceptible to delays in the pragmatic language development that is primarily related to the age of those children and their language abilities, besides their experience in social interactions. These results should be considered in their rehabilitative plan and advocate the importance of early incorporation of pragmatic behaviors into their intervention programs.",{"EN":150},"The effect of The dysphagia handicap index (DHI) is a 25-item self-administered questionnaire. It is a noninvasive tool measuring the handicapping effect of dysphagia in three aspects: the physical, functional, and emotional aspects of life, so a patient can rate the impact of his swallowing problem. The aim of this study is to test the correlation between DHI scores and fibre-optic endoscopic examination of swallowing and the ability of DHI to predict penetration and aspiration. For all patients, FEES examination was done as an assessment tool for dysphagia and DHI. We aimed to evaluate function of the vestibular end organs and vestibular nerve in patients with type 2 DM with polyneuropathy and compare results to those of the patients without polyneuropathy. The participants consisted of three groups: thirty patients with type 2 diabetes mellitus without polyneuropathy (DM), thirty patients with type 2 diabetes mellitus with polyneuropathy (DPN), and thirty healthy non-diabetic individuals as the control group. Clinical examination, videonystagmography, cervical vestibular evoked myogenic potential(cVEMP), ocular vestibular evoked myogenic potential (oVEMP), Gans Sensory Organization Performance (SOP) test, and nerve conduction study were all performed on all groups. P1 and N1 cVEMP latencies and n1 and p1 oVEMP latencies were statistically significantly delayed in both the DM and DPN groups than the control. Also, amplitudes of P1-N1 and n1-p1 were statistically significantly lower in the DM and DPN groups than the control group. DPN patients had longer latencies and lower amplitude of the cVEMP and oVEMP response compared to diabetes patients without neuropathy. There was a statistically significant prevalence of BPPV in the DPN group compared to the control and DM groups. DPN included the following: 4 (13.3%) had vestibular pattern, 12 had polyneuropathic pattern, and 5 (16.7%) had vestibular and polyneuropathic pattern as regards the Gans Sensory Organization Performance (SOP) test. VEMP is considered a promising objective tool in the assessment of the vestibular end organ disorders in patients with type 2 DM with and without polyneuropathy. Our study was conducted on 80 patients who suffer from frontal opacity with sinonasal polyposis and divided randomly into 2 groups: group A: 40 patients candidate for anterior ethmoidectomy without identification of frontal sinus ostium and group B: 40 patients candidate for anterior ethmoidectomy with identification of frontal sinus ostium. The study was conducted in the otolaryngology department  faculty of medicine Cairo university. CT score and sinonasal endoscopy score were statistically significantly improved postoperatively compared to preoperative scores in both groups. There are no significant differences between the 2 groups regarding recurrence rate and complications. Ethmoidectomy without frontal sinusotomy is a potential substitute for frontal sinusotomy for the treatment of chronic frontal sinusitis with sinonasal polyposis, and it can achieve similar This is an indolent tumor known to appear during adult life, with potential for recurrence, but unlikely to metastasize. Although reports of endotheliomas affecting soft tissues, skin, viscera, bone, or lymph nodes have been reported, very few cases of hemangioendothelioma of nasal cavity have come to light, that too in adults only. Authors here-in present an unusual case of hemangioendothelioma of nasal cavity in a 9-year-old girl with a 6-month history of intermittent epistaxis and nasal obstruction. The tumor was excised endoscopically under general anesthesia and histopathology confirmed the diagnosis. Wide excision is the treatment of choice. After preparation of the chitosan patch scaffold (CPS), 60 consecutive dry small and medium sized(from \u003C3 mm up to 5 mm) central tympanic membrane perforations were included in this study. Healing of T.M perforations was achieved in 14 out of 19 in small perforations (73.68%) and 25 out of 41 in medium sized perforations (60.97%) also significant improvement of hearing and tinnitus were achieved post-operatively. Myringoplasty by water insoluble chitosan patch scaffold with 3% chitosan and 3% glycerol is easy, safe and quick method. They are further classified into globoid type and en plaque (aka spheno-orbital meningiomas). These tumours are known for their difficult management because of their invasion to various neurovascular structures and bones. Surgical removal of these tumours is challenging. Complete surgical excision of the tumour is not always guaranteed; hence, intentional incomplete removal sometimes is mandatory to reduce postoperative morbidity. The purpose of this case report is to emphasize and describe the unexplored role of total maxillectomy in the excision of the extensive skull base tumours involving the orbit and infratemporal fossa. In this case report, we are presenting a case of a 50-year-old male diagnosed a case of atypical sphenoid wing meningioma with orbital and paranasal sinus extension who was previously operated on by extracranial approach and presented with the recurrence. Here, we are providing insight and surgical management of this case using total maxillectomy as an approach to access the intracranial approach. Maxillectomy with orbital exenteration can be used as a successful surgical approach to manage skull base lesions and intracranial tumours without any evident post-op complications.",{"EN":936},"An Verbal communication is the highest level of communication, whereas nonverbal (nonsymbolic) communication may remain the only mode of communication in individuals with significant cognitive limitation. Assessment of nonsymbolic skills of two categories of patients with limited verbal output, namely, those with autism [autistic disorder (AD)] and those with Down syndrome (DS), is our main aim. We used the Dynamic Assessment of Nonsymbolic Communication (DANC) test to achieve our aim, because most traditional language tests underestimate this type of communication. Insight into the nonverbal communication abilities of AD and DS patients helps to maintain the communication between them and their caregivers. The current study recruited 20 patients with DS, 20 patients with AD, and 20 normal children as controls. The age of the AD and DS patients ranged from 2 to 5 years, whereas the age of controls ranged from 1 to 2 years. All participants were subjected to detailed personal, family, medical, and developmental history taking. Full clinical examination, hearing tests, and assessment of mental ability (intelligence quotient) were performed. The Childhood Autism Rating Scale was provided to evaluate the severity of autism. The DANC test was administered to evaluate the nonsymbolic skills of all participants. The control group achieved the highest score in all parts of the DANC test, followed by the DS group and the AD group, respectively. There was no significant difference between the DS group and normal group (P>0.05), whereas the autistic group showed highly significantly lower scores compared with both the normal and DS groups (P\u003C0.001). There was also a highly significant difference in scores on the DANC test among the patients in the autistic group in terms of its severity. Application of the DANC test reflected a specific pattern of nonsymbolic skills of AD and DS patients with respect to their relative strengths and weakness. Such patterns should be considered while designing their intervention program aiming either at developing verbal communication or at enhancing the nonverbal skills. Maintaining communication with such patients helps to fulfill their simple needs and prevents frustration among their caregivers caused by lack of communication.",{"EN":1052},"Assessment of Patients with glottic insufficiency attend the phoniatrics clinics with increasing frequency. Glottic insufficiency may be due to vocal-fold paralysis, paresis, atrophy, sulcus vocalis, scarring, and vocal-fold deficiency after laryngeal surgery. A variety of materials exist for injection augmentation. This follow-up study aimed to assess the short-term as well as long-term outcomes of vocal-fold injection with calcium hydroxyapatite (CaHA) in patients with glottic gap 1–3 mm. This study included 25 patients with glottic insufficiency who underwent injection augmentation with CaHA. All patients were scheduled for voice evaluation using a comprehensive subjective and objective battery of assessments. Assessments were performed before the procedure and after the procedure at 2 months and at 1 year. Perceptual voice quality assessed with the GRBAS scale improved and the results were stable after 1 year. Shimmer and HNR improved continuously through the follow-up sessions, while jitter and f0 showed slight improvement after CaHA injection.
