Einbezogen werden in beispielhafter Weise Aspekte der Herausbildung ökologischer Perspektiven im Sinne einer Forschungsprogrammatik, ihre Anerkennung und Rezeption, die Entwicklung entsprechender Theorien und die Berücksichtigung in „Wohn-Studien“. Kontrastiert wird ferner die Entwicklung im deutschsprachigen Raum mit jener in den USA. Es zeigt sich, dass im Zeitraum zwischen den 1930er Jahren und etwa 1960 das Thema Wohnen im Alter im Zuge der Herausbildung einer sozialen Gerontologie vor allem in den USA viel Aufmerksamkeit fand, jedoch ohne die sozial- und verhaltenswissenschaftliche Gerontologie auch theoretisch zu befruchten. Im Zeitraum zwischen 1960 und etwa Mitte der 1980er Jahre stand Wohnen im Alter in den USA in vielen großangelegten Studien im Mittelpunkt, und es wurden Wohntheorien wie die Ökologische Alternstheorie von Lawton vorgeschlagen, welche sich nachhaltig auf die Gerontologie und ihre Forschungsleitbilder auswirkten (Person-Umwelt-Sicht von Altern). In Deutschland fanden in diesem Zeitraum vor allem, wie es häufig hieß, sozialökologische Ansätze profunde Beachtung, wobei das Altern in Institutionen stark im Vordergrund stand. Seit Anfang der 1990er Jahre bestimmte in den USA die Demenzthematik die gerontologische Wohnforschung, während in Deutschland die Vielschichtigkeit des Wohnens in Privathaushalten (wohl teilweise im Sinne eines Nachholeffektes) relativ viel Forschungsaufmerksamkeit auf sich zog. This article is a comparative examination of current and future trends in intergenerational relationships and aging in two societies: the United States and German-speaking countries. While comparing similarities and significant differences between the cultures, we examine five issues related to these trends for families and intergenerational relationships in the 21st century: (1) consequences of population aging for family structures and relationships – the emergence of the beanpole family structure and the longer years of linked lives across generations; (2) the debate about the decline of the family; (3) intergenerational solidarity over the life course; (4) intergenerational conflict across the life course; (5) prospects for solidarity and conflict between generations in the 21st century. Similarities between cultures include the demographic realities of population aging, the shift from pyramids to beanpoles, more years of shared lives between and across generations, more non-traditional family forms, and intergenerational conflicts in families. In contrast to new medicines, exercise programmes are highly complex. This has to be taken into account when designing the research plan. In order to illustrate the development process of a “complex intervention”, we use the example of an exercise programme for community-dwelling, mobility-restricted and chronically ill older adults. Based on a framework for evaluation of complex interventions (Medical Research Council [MRC], UK), a research plan was set up containing the phases: development, feasibility, evaluation, implementation. The development phase resulted in the design of a home-based exercise programme in which the target group is approached and supported via their general practitioner and an exercise therapist. A feasibility study was performed. Three quantitative criteria for feasibility (adoption, safety, continuing participation) were statistically confirmed which permitted the decision to proceed with the research plan. Bestandteil der Interviews von Patienten und Angehörigen war weiter die Nennung der drei aktuellen gesundheitlichen Hauptprobleme der Patienten. Im Verlauf wurden depressive Symptomatik (geriatrische Depressions-Skala) und ADL-Status (Barthel Index) der Patienten bestimmt.    Das allgemeine Krankheitswissen zum Schlaganfall der Angehörigen war besser als das der Patienten. Defizite betrafen insbesondere die Kenntnis individueller Risikofaktoren und der Sekundärprävention. Informationen wurden vorrangig von ärztlichen Mitarbeitern eingeholt. Auch vor Entlassung wurde ein Informationsbedarf vorrangig zu Prognose und Prävention geäußert.    Zwischen den Nennungen der drei wichtigsten aktuellen Gesundheitsprobleme durch Patienten und Angehörige bestand ein hohes Maß an Übereinstimmung, mit der Ausnahme psychischer Probleme, die ausschließlich von Angehörigen genannt wurden. Es war ein signifikanter Anstieg depressiver Symptomatik der Patienten zu verzeichnen. Depressivität war assoziiert mit der Höhe des Barthel-Index-Scores sowie der Selbsteinschätzung des Gesundheitszustandes vor und nach Schlaganfall.    Die Hälfte der Patienten erwarteten bei Aufnahme eine Heilung. Ihr wichtigstes Therapieziel betraf die Fähigkeit gehen zu können. Gemessen am Barthel-Index verbesserten sich die Patienten signifikant im Mittel um 22 Punkte. Patienten wie Angehörige beurteilten den Behandlungserfolg vergleichbar hoch. Die Gesamtbeurteilung der Patientenversorgung durch Angehörige zeigte signifikante Beziehungen zu deren Einzelbeurteilungen pflegerischer, therapeutischer und ärztlicher Versorgung, den Therapieerwartungen sowie dem Erfüllungsgrad eigener Bedürfnisse.    Die Studie zeigt für den Zeitraum stationärer Behandlung einen besonderen Informationsbedarf von Patienten und Angehörigen zur Sekundärprävention. Depression und Krankheitsbewältigung sollten weitere Schwerpunkte einer beratenden Aufklärung sein.
