Cancer cases among Chinese immigrants and Canadian-born Alberta residents were identified from the Alberta Cancer Registry (1974–1993). Incidence rates for Shanghai (1975–1992) were obtained from the International Agency for Research on Cancer (IARC) publications. Direct age-standardized incidence rates (ASIRs) were calculated using the “world standard population.” Descriptive analysis and Poisson regression modelling were employed to obtain the rate ratios for certain cancer sites among the three populations. For males, the overall incidence of cancer (excluding non-melanoma skin cancer) was lowest in Chinese immigrants while being similar in Canadian-born Alberta residents and Chinese living in Shanghai (197 vs. 224 and 232/100,000). For females, the overall incidence in Chinese immigrants was lower than Canadian-born Alberta residents but similar to that in Chinese living in Shanghai (154 vs 200 and 150/100,000). For cancers that are common in China (stomach and esophagus), the incidence rates for Chinese immigrants were more similar to those for Canadian-born residents than to rates for Shanghai. However, the incidence of liver cancer was very high in the immigrants, suggesting the possible presence of an initiating event during childhood or early adulthood. For cancers that are traditionally uncommon in China (breast and prostate), rates for immigrants were mid-way between those of the two comparison groups. This study supports observations that the risk of cancer in immigrants tends towards the risk of people in the new host country.
Cancer Incidence Patterns Among Chinese Immigrant Populations in Alberta
Geddes M, Parkin DM, Khlat M, Balzi D, Buiatti E: Cancer in Italian Migrant Populations. Data from a structured questionnaire administered to a random sample of 206 Latinos was analyzed using multivariate logistic regression. Qualitative data served to explain quantitative results. Point estimates for various access measures were similar to national data. In multivariate logistic regression, income and education determined having health insurance (OR 6.8 and 7.4; 95% CI 2.7–17.3 and 2.9–19.0, respectively). Time in the U.S. and health insurance determined having a regular source of care (OR 4.6 and 5.8; 95% CI 1.7–12.8 and 2.1–16.0, respectively). Yet too little is known about perceptions influencing their health. This study investigated gender and acculturation differences in TB-specific Health Belief Model (HBM) constructs, and the applicability of the HBM's traditional configuration to Mexican Americans. Acculturation and gender substantially influenced the findings. Traditional Mexican Americans reported higher perceived susceptibility and seriousness, more barriers, and greater attention to cues regarding TB prevention than Highly Integrated Biculturals. Women reported greater benefits, attention to cues, and intent to engage in TB prevention behaviors than men. Highly Integrated Bicultural men reported less attention to cues and less intent to engage in health behaviors than other groups. The traditional HBM configuration did not fit this sample. Reconfiguration did, however, result in adequate fit. Overall, higher perceived susceptibility, action benefits, attention to media cues, and female gender predicted greater intent to engage in TB health behaviors.
Acculturation and Health Beliefs of Mexican Americans Regarding Tuberculosis Prevention
Cauthen, GM, Pio, HG, Ten Dam, HG: Annual risk of tuberculosis infection. Significant concern has been raised regarding the ability of immigrant women to access prenatal care services because of severe restrictions imposed on immigrants’ eligibility for Medicaid-funded services following the passage in 1996 of the federal Personal Responsibility and Work Opportunity Reform Act (PRWORA) and the Illegal Immigration Reform and Immigrant Responsibility Act (IIRAIRA). We conducted an interview-based study of prenatal care utilization with women of Mexican ethnicity and diverse immigration statuses in San Diego County, California. Our findings indicate that, despite increased levels of fear associated with recent immigration and with undocumented status, there were no statistically significant differences across immigration statuses in length of time to receipt of medical care for gynecological events and for prenatal care.
Welfare and Immigration Reform and Use of Prenatal Care Among Women of Mexican Ethnicity in San Diego, California
Currie J, Grogger J. Building upon past research that has explored the role of economic remittances in contributing to the positive relationship between international migration and birth outcomes in sending country communities, the present analysis focuses on the noneconomic effects of paternal migration experience on infant health. The data come from a hospital-based postpartum survey (HPS 2001) recently completed in two high-migrant sending states of Western Mexico. The findings demonstrate that the positive effect of international migration on infant health occurs in the context of considerable loss of social support and high stress levels. The project conducted 87 qualitative interviews and nine focus groups to develop a culturally tailored intervention to improve Pap testing rates. The intervention consisted of an educational/motivational video, a pamphlet, and home visits. Less acculturated Chinese women draw on a rich tradition of herbal knowledge and folk practices historically based on Chinese medical theory, now mixed with new information from the media and popular culture. The video, the pamphlet, and the outreach workers knowledge base were designed using these results and combined with biomedical information to address potential obstacles to Pap testing. Culturally relevant information for reproductive health promotion was easily retrieved through qualitative interviews and used to create educational materials modeling the integration of Pap testing into Chinese women's health practices.
Development of Cervical Cancer Control Interventions for Chinese Immigrants
U.S. Census Bureau: Census 2000 Profile
Costa R, Renaud V: The Chinese in Canada. study assessed levels of HIV knowledge and identified factors associated with HIV knowledge among a sample of heterosexual Puerto Rican and Mexican men and women, ages 18 to 45. The sample consisted of 144 men and women living in San Diego County, California, who self-identified as being of Mexican ethnicity and 209 men and women living in Cuyahoga County, Ohio, who self-identified as Puerto Rican. Interviews were conducted by trained, bilingual interviewers. Data were collected on demographic variables, attitudes towards decision-making in relationships, and HIV knowledge and risk behaviors. Puerto Rican individuals were significantly more likely than Mexican individuals to respond correctly to almost one-half of the 12 HIV knowledge items. Multiple logistic regression analysis indicated that higher levels of education, greater U.S. acculturation, legal status and birth in the United States, a self-focused locus of control in relationships, and being male were predictors of higher knowledge.
HIV Knowledge Among a Sample of Puerto Rican and Mexican Men and Women
Aruffo J, Coverdale J, Vallbona C: AIDS knowledge in low income and minority populations. in 1975, Mien refugees migrated to the United States from the highlands of Southeast Asia, primarily but not exclusively from Laos. Reported here are 1993 data on the basic size and composition of the Iu-Mien population in the West Coast states (Washington, Oregon, and California), the most popular settlement destination for this group. Three-fourths of Mien in this region live in California. Surveys reveal a young population, half being under age 15 years but with a surprisingly large (6.8%) proportion of people aged 60 or older. Approximately half of all Mien households, which typically contain 5–7 members, are two-generation nuclear families, i.e., parents and their school-age or younger children. Through the addition of grandparents, other kin, or unrelated individuals, the remaining households comprise three- or four-generation domestic units. Most (around 90%) Mien households are headed by middle-aged males, although some households in all locations are headed by females. Employment rates for Mien households vary by state and by location in an urban or rural area. State welfare policies as well as differential availability of work for unskilled or poorly educated individuals seem to be major factors accounting for differences in employment.
A Demographic Survey of Iu-Mien in West Coast States of the U.S., 1993
Waters T.
