Endocarditis involved a native or prosthetic valve in six and four of those patients, respectively. None had a concurrent or preceding history of colon disease and only one had subclinical chronic liver disease. Nghiên cứu này nhằm đánh giá hiệu quả và độ an toàn của ESD cho LST‐DL.\u003C\u002Fjats:p>\u003C\u002Fjats:sec>\u003Cjats:sec>\u003Cjats:title>Phương pháp\u003C\u002Fjats:title>\u003Cjats:p>Các bệnh nhân liên tiếp có LST‐DL đã trải qua ESD tại bệnh viện của chúng tôi từ tháng 1 năm 2010 đến tháng 12 năm 2015 đã được ghi nhận hồi cứu trong nghiên cứu này. Tỷ lệ cắt bỏ \u003Cjats:italic>en bloc\u003C\u002Fjats:italic> (93.9% [46\u002F49] so với 94.8% [91\u002F96]) và tỷ lệ cắt bỏ \u003Cjats:italic>en bloc\u003C\u002Fjats:italic> R0 (83.7% [41\u002F49] so với 88.5% [85\u002F96]) cho LST‐DL và LST‐NDL, không có sự khác biệt đáng kể. This study aimed to evaluate the efficacy and safety of ESD for LST‐DL.\u003C\u002Fjats:p>\u003C\u002Fjats:sec>\u003Cjats:sec>\u003Cjats:title>Methods\u003C\u002Fjats:title>\u003Cjats:p>Consecutive patients with LST‐DL who had undergone ESD at our hospital between January 2010 and December 2015 were retrospectively enrolled in this study. Rates of \u003Cjats:italic>en bloc\u003C\u002Fjats:italic> resection, R0 resection, and complications, pathological characteristics, and tumor recurrence were analyzed and compared with those of LST in the rectum not involving the dentate line (LST‐NDL).\u003C\u002Fjats:p>\u003C\u002Fjats:sec>\u003Cjats:sec>\u003Cjats:title>Results\u003C\u002Fjats:title>\u003Cjats:p>Altogether 49 patients with LST‐DL (median age 63 years; 39 women; median lesion size 57 mm; median follow‐up period of 24 months) and 96 patients with LST‐NDL (median age 67 years; 31 women; median lesion size 47 mm; median follow‐up period of 31 months) were enrolled. \u003Cjats:italic>En bloc\u003C\u002Fjats:italic> resection (93.9% [46\u002F49] vs 94.8% [91\u002F96]) and \u003Cjats:italic>en bloc\u003C\u002Fjats:italic> R0 resection rates (83.7% [41\u002F49] vs 88.5% [85\u002F96]), respectively, for LST‐DL and LST‐NDL, with no significant differences. However, ESD for LST‐DL had a longer procedure time (77 min vs 54 min, \u003Cjats:italic>P\u003C\u002Fjats:italic> = 0.02), a greater postprocedural perianal pain rate (28.6% vs 0%, \u003Cjats:italic>P\u003C\u002Fjats:italic> &lt; 0.001), and more anal strictures (4.1% vs 0%, \u003Cjats:italic>P\u003C\u002Fjats:italic> = 0.04). The complication rates of perforation, bleeding and fever, recurrence rate, and pathological characteristics did not differ between the two groups.\u003C\u002Fjats:p>\u003C\u002Fjats:sec>\u003Cjats:sec>\u003Cjats:title>Conclusions\u003C\u002Fjats:title>\u003Cjats:p>ESD is a safe and effective therapeutic modality for LST‐DL. Screening of the four target single nucleotide polymorphisms (SNPs), including rs3807306, rs4728142, rs10954213 and rs11770589 was carried out in a real‐time polymerase chain reaction (\u003Cjats:styled-content style=\"fixed-case\">PCR\u003C\u002Fjats:styled-content>) thermal cycler using \u003Cjats:styled-content style=\"fixed-case\">TaqMan\u003C\u002Fjats:styled-content> genotyping assay. The heterozygous \u003Cjats:styled-content style=\"fixed-case\">A\u002FG\u003C\u002Fjats:styled-content> genotype of rs10954213 was significantly associated with CD (OR 4.319, \u003Cjats:italic>P\u003C\u002Fjats:italic> = 0.0377). On the other hand, the homozygous \u003Cjats:styled-content style=\"fixed-case\">A\u003C\u002Fjats:styled-content> and heterozygous \u003Cjats:styled-content style=\"fixed-case\">A\u002FG\u003C\u002Fjats:styled-content> genotypes of the rs11770589 were significant in the controls (OR 0.4242, \u003Cjats:italic>P\u003C\u002Fjats:italic> = 0.0166) and patients (OR 2.000, \u003Cjats:italic>P\u003C\u002Fjats:italic> = 0.0179), respectively. In the ethnic‐stratification analysis, the rs11770589 homozygous A genotype was protective in Indians (OR 0.1551, \u003Cjats:italic>P\u003C\u002Fjats:italic> = Esophageal adenocarcinoma and esophageal squamous cell carcinoma are the most prevalent types. As epidemiological studies report that ingesting hot substances is one major risk factor for squamous cell carcinoma, evaluating the effect of this external stress on esophagus cells seems desirable. This specific kind of stress brings about cellular changes and stabilizes them by affecting different cellular features such as genetic stability, membrane integrity and the regulation of signaling pathways. It also causes tissue injury by affecting the extracellular matrix and cell viability. Thus, one of the main consequences of thermal injury is the activation of the immune system, which can result in chronic inflammation. The genetic alteration that has occurred during thermal injury and the consequent reduction in the function of repair systems is further strengthened by chronic inflammation, thereby increasing the probability that mutated cell lines may appear. The molecules that present in this circumstance, such as heat shock proteins, cytokines, chemokines and other inflammatory factors, affect intercellular signaling pathways, including nuclear factor kappa‐light‐chain‐enhancer of activated B cells, signal transducer activator of transcription‐3 and hypoxia‐inducible factor 1α in supporting the survival and emergence of mutant phenotypes and the consequent malignant progression in altered cell lines. Pooled effect sizes with 95% confidence interval (\u003Cjats:styled-content style=\"fixed-case\">CI\u003C\u002Fjats:styled-content>) were calculated using a random effects model. All the samples were tested for anti‐HCV antibodies and all seropositive samples were further tested for HCV RNA by polymerase chain reaction (PCR).\u003C\u002Fjats:p>\u003Cjats:p>\u003Cjats:bold>RESULTS: \u003C\u002Fjats:bold> Of the total 6817 serum samples tested, 998 (14.63%) were positive for anti‐HCV antibodies. HCV RNA PCR was detected in 494 (49.50%) anti‐HCV‐positive samples. The prevalence of anti‐HCV antibodies were significantly higher in males (15.09%) than in females (12.3%) (\u003Cjats:italic>P &lt;\u003C\u002Fjats:italic> 0.009). A significant difference was also noted in the anti‐HCV prevalence rate among different age groups tested (\u003Cjats:italic>P &lt;\u003C\u002Fjats:italic> 0.01). In a multivariate logistic regression analysis, injected drug use (adjusted \u003Cjats:italic>OR\u003C\u002Fjats:italic> 6.6 [95%\u003Cjats:italic>CI\u003C\u002Fjats:italic> 4.1–9.9]), blood transfusion (adjusted \u003Cjats:italic>OR\u003C\u002Fjats:italic> 5.9 [95%\u003Cjats:italic>CI\u003C\u002Fjats:italic> 2.9–12.3]), pricked with a needle (adjusted \u003Cjats:italic>OR\u003C\u002Fjats:italic> 2.2 [95%\u003Cjats:italic>CI\u003C\u002Fjats:italic> 1.6–3.1]), re‐use of syringes (adjusted \u003Cjats:italic>OR\u003C\u002Fjats:italic> 1.7 [95%\u003Cjats:italic>CI\u003C\u002Fjats:italic> 0.8–3.6]) and being over 35 years old (adjusted \u003Cjats:italic>OR\u003C\u002Fjats:italic> 1.3 [95%\u003Cjats:italic>CI\u003C\u002Fjats:italic> 0.9–1.9]) were independent risk factors for HCV infection.\u003C\u002Fjats:p>\u003Cjats:p>\u003Cjats:bold>CONCLUSION: \u003C\u002Fjats:bold> The study showed a high seroprevalence of anti‐HCV antibodies in a general and apparently healthy population of the Punjab province of Pakistan.
