First, in a randomly selected set of 49 biopsies, we examined the effect of additional interpretation of p16\u003Cjats:sup>INK4a\u003C\u002Fjats:sup>immunostained slides, on the agreement of CIN diagnosis among three pathologists. Second, the full series of samples was used to assess the accuracy of p16\u003Cjats:sup>INK4a\u003C\u002Fjats:sup>-supported lesion grading by a single pathologist, by evaluating the degree of diagnostic agreement with the consensus diagnosis of expert pathologists based on H&amp;E-stained sections only.\u003C\u002Fjats:p>\u003C\u002Fjats:sec>\u003Cjats:sec>\u003Cjats:title>Results\u003C\u002Fjats:title>\u003Cjats:p>The study shows that the interobserver agreement between three pathologists for the routine H&amp;E-based diagnosis ranged from fair (weighted kappa 0.44 (95% CI 0.19 to 0.64)) to moderate (weighted kappa 0.66 (95% CI 0.47 to 0.79)). The concordance increased substantially for p16\u003Cjats:sup>INK4a\u003C\u002Fjats:sup>-supported grading (mean weighted kappa 0.80 (95% CI 0.66 to 0.89)). Microscopically, the tumour was composed of a major medullary portion and a minor tubular adenocarcinoma which had invaded the myometrium, the myometrial lymphatics and blood vessels. Neoplastic cells in the medullary portion were polygonal with glycogen-rich cytoplasm. Vascular permeation by neoplastic cells was prominent. Extensive hepatoma-like features were observed. The tumour cells lacked features suggestive of a diagnosis of embryonal carcinoma or endodermal sinus tumour. The production of AFP by the tumour cells was demonstrated immunohistochemically using the PAP technique. The minimal dose that altered a platelet function test and the persistence of this alteration at different dose levels were studied. Minute doses of indomethacin (0·035 mg\u002Fkg) were effective but the effect of even a large single dose did not persist. A tenth of the therapeutic dose of aspirin (1 mg\u002Fkg) was effective, and higher doses altered the platelets' function for several days. Benorylate in a high therapeutic dose gave aspirin-like results. Paracetamol and sodium salicylate were relatively inactive. The persistence of the aspirin effect may be related to the acetyl group. Twist protein level in one immortalised normal oesophageal epithelial cell line and six oesophageal SCC cell lines was measured by western blot analysis.\u003Cjats:italic>Twist\u003C\u002Fjats:italic>mRNA level in 30 pairs of frozen specimens of primary oesophageal SCC and non-neoplastic oesophageal epithelium from the upper resection margin of corresponding oesophagectomy specimen was also determined by semiquantitative reverse transcription-PCR.\u003C\u002Fjats:p>\u003Cjats:p>\u003Cjats:bold>Results:\u003C\u002Fjats:bold>It was found that Twist was upregulated in oesophageal SCC cell lines, and its mRNA and protein levels were both increased in oesophageal SCC and the non-neoplastic oesophageal epithelium (p&lt;0.001).
