Die Suche nach effektiven Präventionsmaßnahmen und Optimierungen im Geburtsmanagement beschäftigt die Forschung daher seit vielen Jahrzehnten. Die Hauptursachen der Beckenbodendysfunktion nach einer Geburt sind dabei die Verletzung des Beckenbodens durch Kompression, Zerrungen, Ab- und Ausriss von Nerven, Muskeln und Bindegewebsfasern. Der Abriss des M. levator ani spielt dabei in der Entstehung von Inkontinenz und Prolaps eine wichtige Rolle. Vielfach wird die Entbindung per Sectio caesarea als effektive Prävention zur Verhinderung von Harninkontinenz und Prolaps angesehen, ein Blick auf die aktuelle Datenlage bestätigt diese These jedoch nicht. Die Sequenzierung der DNA ermöglicht die Identifizierung von Bakterien oder ihrer Fragmente im Harn von gesunden und symptomatischen Patientinnen. Unsere Arbeit bietet einen Überblick über die derzeitige Studienlage zum Mikrobiom der weiblichen Harnwege (MHW), beschreibt die zu seiner Identifikation erforderlichen molekularbiologischen Methoden und fasst seine Assoziation mit urogynäkologischen Erkrankungen zusammen. Auf der Grundlage von 16S-rRNA-Sequenzanalysen wurde das Paradigma der Sterilität des Harns widerlegt. Das MHW von unter Dranginkontinenz, hyperaktiver Blase, Stressinkontinenz, Schmerzblase und wiederkehrenden Harnwegsinfekten leidenden Frauen unterscheidet sich in Diversität und Reichtum von jenem gesunder Individuen. Diese Erkentnisse sowie ihre klinische Relevanz müssen noch in der zu erwartenden großen Zahl zukünftiger Studien bestätigt werden. Probiotika haben ein therapeutisches Potenzial und könnten zum besseren Verständnis sowohl der physiologischen als auch der pathologischen Wirts-Pathogen-Beziehungen im weiblichen Harn Zu den häufigsten sexuellen Beschwerden nach gynäkologischen Karzinomen zählen Libidoverlust und Dyspareunie. Zur Erfassung der sexuellen Gesundheit nach onkologischen Erkrankungen wird von der „European Organisation for Research and Treatment of Cancer – Sexual Health Questionnaire“ (EORTC SHQ-22) entwickelt. Ausgehend von der Literatur wurde eine Liste von 52 sexuellen Problemen erstellt und in folgende Bereiche unterteilt: sexuelle Aktivität, sexuelle Funktionsfähigkeit, Auswirkungen der Therapien auf die Sexualität, Intimität, Ängste bezogen auf die Sexualität, Kommunikation\u002FBeziehung, Stress bezogen auf die Sexualität und sexuelle Bedürfnisse. Diese Liste wurde von 107 Patienten und 83 onkologischen Behandlern aus 12 Ländern hinsichtlich Relevanz und Priorität auf einer Skala von 1–4 beurteilt. Die Ergebnisse zeigten signifikante Unterschiede zwischen den Beurteilungen der Sexualität durch die Betroffenen und deren Behandler. Insgesamt beurteilten Behandler die Relevanz von mehr als der Hälfte (32 von 52) der Problembereiche signifikant höher als Betroffene (p \u003C 0,001). Die Einschätzungen der Behandler lagen in den Kategorien Auswirkungen der Therapien auf die Sexualität, Ängste bezogen auf die Sexualität und sexuelle Bedürfnisse deutlich höher als die der Betroffenen. Mit dem EORTC-SHQ22-Fragebogen steht demnächst ein Screeninginstrument zur Verfügung, mit dem Menschen mit sexuellen Problemen identifiziert werden können. Damit können frühzeitig eine adäquate sexualmedizinische Beratung und Therapie eingeleitet werden, um sexuellen Funktionsstörungen vorzubeugen bzw. diese zu
