Sie stellen einen bedeutenden Risikofaktor für kardiovaskuläre Erkrankungen dar. Die Diagnostik von schlafbezogenen Atmungsstörungen kann mit einer gezielten schlafmedizinischen Anamnese und spezifischen Fragebögen beginnen und erfordert erst im zweiten Schritt einen apparativen Aufwand zur Erfassung der Sauerstoffsättigung, der Herzfrequenz, des Atmungsflusses und der Atmungsanstrengung. Die ambulant verfügbare Methodik gibt bereits gute Hinweise auf den Schweregrad der Atmungsstörung und erlaubt die Weiterleitung des Patienten zur weiterführenden Differenzialdiagnostik und\u002Foder unmittelbaren Initiierung einer adäquaten Therapie. Um eine schlafbezogene Atmungsstörung auszuschließen oder zwischen einer obstruktiven und zentralen Schlafapnoe zu differenzieren bzw. das Vorliegen weiterer Schlafstörungen zu diagnostizieren, ist eine Untersuchung im Schlaflabor mit kardiorespiratorischer Polysomnographie erforderlich. Sie ist auch bei Patienten mit Komorbiditäten indiziert. Aufgrund der hohen Erfolgsrate und geringen Komplikationsquote ist die interventionelle Therapie in den weit überwiegenden Fällen die Methode der Wahl. Die Behandlungsstrategie sollte interdisziplinär abgestimmt werden und idealerweise innerhalb von Gefäßzentren erfolgen. Das wesentliche Problem bei Interventionen ist das Auftreten von Restenosen, insbesondere bei infrapoplitealen Läsionen. Einen deutlichen Fortschritt stellen medikamentenbeschichtete Ballonkatheter dar, die in der femoropoplitealen Strombahn die Notwendigkeit für Reinterventionen drastisch reduzieren konnten. A systematic review and meta-analysis of all published articles from Embase, Pubmed\u002FMEDLINE, and Ovid was carried out. In all, 10 studies including 52,702 patients were identified. The pooled estimate for the overall event rate was 0.15, and cardiovascular causes were the main reason for 30-day readmission (0.42, 95% confidence interval [CI]: 0.39–0.45). In addition, the pooled incidence of 1‑year readmission was 0.31, and cardiovascular events were still the main cause (0.41, 95% CI: 0.33–0.48). Patients with major and life-threatening bleeding, new permanent pacemaker implantation, and clinical heart failure were associated with a high risk for early readmission after TAVI. Moreover, an advanced (≥3) New York Heart Association classification, acute kidney injury, paravalvular leak, mitral regurgitation (≥ moderate), and major bleeding predicted unfavorable outcome to 1‑year readmission. Female gender and transfemoral TAVI was associated with a lower risk for unplanned rehospitalization. This meta-analysis found cardiovascular factors to be the main causes for both 30-day and 1‑year rehospitalization. Heart failure represented the most common cardiovascular event at both short-term and mid-term follow-up. Several baseline characteristics and procedure-related factors were deemed unfavorable predictors of readmission. The myocardial performance index (MPI), which combines parameters of both systolic and diastolic ventricular function, provides a better marker of left ventricular dysfunction in hypertensive patients with preserved left ventricular ejection fraction (LVEF). We aimed to investigate the relationship between hs-cTnT and tissue Doppler-derived MPI in newly diagnosed hypertensive patients with preserved LVEF. We studied 306 patients with newly diagnosed hypertension. The patients were divided into two groups according to their median MPI values: MPIlo and MPIhigh groups. The MPI was calculated from the pulsed-wave Doppler recordings. Left ventricular mass index (LVMI) was determined according to the Devereux formula. hs-cTnT, uric acid, and high-sensitivity C-reactive protein (hs-CRP) levels were measured in all patients. Hs-cTnT, hs-CRP, and uric acid levels were higher in the MPIhigh group compared with the MPIlow group (p &lt; 0.05, for all). The LVMI values in the MPIhigh group were higher than in the MPIlow group (p &lt; 0.05). MPI was independently associated with age, uric acid, hs-cTnT, and LVMI in multivariate analysis. Multivariate logistic regression analysis showed that MPI and LVMI were independent predictors of high hs-cTnT level in newly diagnosed hypertensive patients with preserved LVEF. Multivariate logistic regression Cox models were used to analyze demographic, echocardiographic and exercise variables in order to determine independent predictors of cardiovascular mortality and rehospitalization. LVEF failed to show prognostic power (hazard ratio [HR] 0.99 [95% confidence interval, CI, 0.94–1.03]; p = not significant), whereas moderate to severe mitral regurgitation (HR, 5.71 [95% CI 1.75–18.6]; p = 0.004) and atrial fibrillation (HR 1.67 [95% CI 1.15–2.44]; p = 0.008) were associated with a poorer prognosis. This is not only difficult in patients with acute coronary syndrome (ACS) but also in the large number of patients with AF undergoing elective percutaneous coronary intervention (PCI). The challenge is to strike a balance between the increasing risk of bleeding events and ischemic or thrombotic events. Until recently, guidelines were based on expert consensus and a few small, many of them retrospective, trials. A so-called triple therapy with a vitamin K antagonist (VKA) and dual antiplatelet therapy (DAPT) with aspirin and clopidogrel was recommended for patients with AF undergoing PCI in stable coronary artery disease or for those with ACS.
