Records of 364,273 NIS patients were used to corroborate time trends.\u003C\u002Fjats:p>\u003Cjats:p>\u003Cjats:bold>Principal Findings. \u003C\u002Fjats:bold> There were no significant differences in AMI mortality among insured patients in New Jersey relative to New York or the NIS. However, there was a relative increase in mortality of 41 to 57 percent among uninsured New Jersey patients post‐reform, and their rates of expensive cardiac procedures decreased concomitantly.\u003C\u002Fjats:p>\u003Cjats:p>\u003Cjats:bold>Conclusions. \u003C\u002Fjats:bold> The introduction of hospital price competition and reductions in subsidies for hospital care of the uninsured were associated with an increased mortality rate among uninsured New Jersey AMI patients. A relative decrease in the use of cardiac procedures in New Jersey may partly explain this finding. Regression‐adjusted changes were obtained from logistic regression models controlling for demographic and socioeconomic characteristics.\u003C\u002Fjats:p>\u003C\u002Fjats:sec>\u003Cjats:sec>\u003Cjats:title>Principal Findings\u003C\u002Fjats:title>\u003Cjats:p>Uninsurance declined for both sexual minority adults (5 percentage points, \u003Cjats:italic>P\u003C\u002Fjats:italic> &lt; .05) and heterosexual adults (2.5 percentage points, \u003Cjats:italic>P\u003C\u002Fjats:italic> &lt; .001) between 2013‐2014 and 2017‐2018. Reductions in uninsurance for sexual minority and heterosexual adults were associated with increases in Medicaid coverage. Sexual minority and heterosexual adults were also less likely to report unmet medical care in 2017‐2018 compared with 2013‐2014. Low‐income adults (regardless of sexual minority status) experienced relatively large increases in Medicaid coverage and substantial improvements in access to care over the study period. We then recoded the same charts using ICD‐9‐CM and determined the agreement between the ICD‐9‐CM data and chart data for recording those same conditions. The accuracy of ICD‐10 data relative to chart data was compared with the accuracy of ICD‐9‐CM data relative to chart data.\u003C\u002Fjats:p>\u003Cjats:p>\u003Cjats:bold>Results. \u003C\u002Fjats:bold> Sensitivity values ranged from 9.3 to 83.1 percent for ICD‐9‐CM and from 12.7 to 80.8 percent for ICD‐10 data. Positive predictive values ranged from 23.1 to 100 percent for ICD‐9‐CM and from 32.0 to 100 percent for ICD‐10 data. Specificity and negative predictive values were consistently high for both ICD‐9‐CM and ICD‐10 databases. Of the 32 conditions assessed, ICD‐10 data had significantly higher sensitivity for one condition and lower sensitivity for seven conditions relative to ICD‐9‐CM data. The two databases had similar sensitivity values for the remaining 24 conditions.\u003C\u002Fjats:p>\u003Cjats:p>\u003Cjats:bold>Conclusions. \u003C\u002Fjats:bold> The validity of ICD‐9‐CM and ICD‐10 administrative data in recording clinical conditions was generally similar though validity differed between coding versions for some conditions. The implementation of ICD‐10 coding has not significantly improved the quality of administrative data relative to ICD‐9‐CM. \u003C\u002Fjats:bold> We report the validation of an instrument to measure mistrust of health care organizations and examine the relationship between mistrust and health care service We surveyed 401 persons and followed up with 327 persons (81.5 percent) 3 weeks after the baseline interview. We conducted tests of the validity and reliability of the Medical Mistrust Index (MMI) and then conducted multivariate modeling to examine the relationship between mistrust and five measures of underutilization of health services.\u003C\u002Fjats:p>\u003Cjats:p>\u003Cjats:bold>Results. \u003C\u002Fjats:bold> Using principle components analysis, we reduced the 17‐item MMI to 7 items with a single dimension. Test–retest reliability was moderately strong, ranging from Pearson correlation of 0.346–0.697. In multivariate modeling, the MMI was predictive of four of five measures of underutilization of health services: failure to take medical advice (\u003Cjats:italic>b\u003C\u002Fjats:italic>=1.56, \u003Cjats:italic>p\u003C\u002Fjats:italic>&lt;.01), failure to keep a follow‐up appointment (\u003Cjats:italic>b\u003C\u002Fjats:italic>=1.11, \u003Cjats:italic>p\u003C\u002Fjats:italic>=.01), postponing receiving needed care (\u003Cjats:italic>b\u003C\u002Fjats:italic>=0.939, \u003Cjats:italic>p\u003C\u002Fjats:italic>=.01), and failure to fill a prescription (\u003Cjats:italic>b\u003C\u002Fjats:italic>=1.48, \u003Cjats:italic>p\u003C\u002Fjats:italic>=.002). This relationship was significant, albeit reduced in magnitude, even while controlling for macro‐level (GDP per capita, total physicians per one thousand population, percent of elderly) and micro‐level (average number of ambulatory care visits, per capita income, alcohol and tobacco consumption) determinants of population health.\u003C\u002Fjats:p>\u003Cjats:p>\u003Cjats:bold>Conclusions.\u003C\u002Fjats:bold>(1) Strong primary care system and practice characteristics such as geographic regulation, longitudinality, coordination, and community orientation were associated with improved population health. (2) Despite health reform efforts, few OECD countries have improved essential features of their primary care systems as assessed by the scale used here. Multivariate regression methods were used to analyze the influence of market and organizational factors on the payment method developed by physician organizations for individual physicians.\u003C\u002Fjats:p>\u003Cjats:p>\u003Cjats:bold>Principal Findings. \u003C\u002Fjats:bold> Within physician organizations, approximately one‐quarter of physicians are paid on a purely retrospective (fee‐for‐service) basis, approximately one‐quarter are paid on a purely prospective (capitation, nonproductivity‐based salary) basis, and approximately one‐half on blends of retrospective and prospective methods. Clinical performance measures for women's health screening rates, child and adolescent immunization rates, heart disease screening rates, diabetes screening rates, and smoking cessation were developed from HEDIS\u003Cjats:sup>®\u003C\u002Fjats:sup> data. Measures of patient perceptions of care are obtained from CAHPS\u003Cjats:sup>®\u003C\u002Fjats:sup> survey data submitted as Healthplan Employer Data and Information Set, Consumer Assessment of Health Plans 2.0 H.\u003C\u002Fjats:p>\u003Cjats:p>\u003Cjats:bold>Study Design. \u003C\u002Fjats:bold> Multivariate regression cross‐sectional analysis of 272 health plans was used to evaluate the relationship of health plan characteristics with measures of clinical performance and patient perceptions of care.\u003C\u002Fjats:p>\u003Cjats:p>\u003Cjats:bold>Principal Findings. \u003C\u002Fjats:bold> The form of delivery system, measured by percent of care delivered by staff and group model systems, is significantly related (\u003Cjats:italic>p\u003C\u002Fjats:italic>≤.05) with four of the five clinical performance indices but none of the three satisfaction performance indices. Other variables significantly associated with performance were being geographically located in the Northeast, having nonprofit status, and for patient satisfaction, not being part of a larger insurance company.\u003C\u002Fjats:p>\u003Cjats:p>\u003Cjats:bold>Conclusions. \u003C\u002Fjats:bold> These comparative results provide evidence suggesting that the type of delivery system used by health plans is related to many clinical performance measures but is not related to patient perceptions of care.
