Axonal sensitization to heat by inflammatory mediators has been demonstrated and shown to depend on the heat‐ and proton‐activated ion channel TRPV1. Recently, we have demonstrated \u003Cjats:italic>in vitro\u003C\u002Fjats:italic> that heat stimulation of nociceptive axons generates ectopic action potential discharge which resembles the heat response of the corresponding cutaneous nerve endings. It remained however, to be established whether adequate axonal stimulation could also generate projected sensations in a conscious human subject. In a singular human trial, the superficial radial nerve (SR) was exposed and stimulated mechanically as well as with noxious cold (3°C). These stimuli were unable to induce any conscious local or projected sensations. However, controlled radiant heat applied to the nerve resulted in intense slowly adapting burning pain sensations projected into the center of the SR innervation area. No local sensation was reported. Thus, presumably activated \u003Cjats:italic>nervi nevorum\u003C\u002Fjats:italic> in the sheath of a healthy nerve do not cause conscious sensations, while axons of passage in mid‐nerve exhibit a sensory transduction capacity for noxious heat though not for mechanical and cold stimulation. This measure was constructed by extracting 20 items from its 40‐item parent measure (PASS). Initial studies of the PASS‐20 suggest that the psychometric properties have been preserved. The purpose of the present study extended this research and explored the factor structure of the PASS‐20, and its reliability and validity in a sample of pain patients receiving treatment in a community physiotherapy clinic. Patients with current pain (\u003Cjats:italic>n\u003C\u002Fjats:italic>=201) were asked to complete a battery of self‐report measures related to the experience of pain on two separate occasions (3‐month interval). Results of principal components analyses suggested that a 4‐factor solution representing fear of pain, escape‐avoidance, physiological symptoms, and cognitive symptoms of anxiety provided the best fit to these data. Results also showed that the total and subscale scores of the PASS‐20 have good reliability (internal consistency, test—retest) and validity (construct) correlating greater with other conceptually similar measures than distinct constructs. These results suggest that this measure has good utility for both clinical and research applications. While in many patients the cause of vulvodynia remains uncertain, we and others have previously shown increased intraepithelial and papillary innervation in vulvodynia. The vanilloid receptor VR1 (TRPV1) is expressed by nociceptors, and is triggered by capsaicin, noxious heat, protons, and chemicals produced during inflammation. In the present study we show increased papillary VR1 fibres by immunostaining and image analysis in vulvodynia tissues compared to controls (\u003Cjats:italic>P\u003C\u002Fjats:italic>&lt;0.002). VR1 expression was found to be significantly increased when the percentage area immunostained was expressed as a ratio of VR1 to PGP 9.5, a pan‐neuronal marker (\u003Cjats:italic>P\u003C\u002Fjats:italic>=0.01). VR1‐positive fine epidermal fibres also appeared to be increased in vulvodynia tissues, by inspection. Fibres immunoreactive to the voltage‐gated sodium channel SNS1\u002FPN3 (Nav1.8), also expressed by nociceptors, were relatively scarce in both vulvodynia and control tissues. We hypothesize that increased expression of VR1 by nociceptors could mediate some of the symptoms in vulvodynia, for which systemic or topical specific VR1 antagonists may provide novel MRIs were acquired and joint‐related RDC\u002FTMD diagnoses were modified on the basis of MRI findings. Group I suffered from joint pathology, group II from myofascial pain and group III from a combination. Non‐parametric statistics were used to reveal group‐related differences in psychosocial and behavioural aspects. Differences in group assignment based on clinical and MRI examination were assessed (κ statistics).\u003C\u002Fjats:p>\u003Cjats:p>\u003Cjats:italic>Results\u003C\u002Fjats:italic>: MRI findings led to group assignments that were different from clinical assignments (\u003Cjats:italic>k\u003C\u002Fjats:italic> = 0.3). For both types of assignment, patients with myofascial pain displayed higher somatization scores, used more splints and consulted more physicians. For clinical assignment only, depression scores were higher in the myofascial pain group. However, more extensive surgical procedures with higher baseline pain intensity did not appear to influence the ability to document the additive analgesic effect of codeine when given with paracetamol, partly due to large interindividual variation in baseline pain intensity. The aim of the present study was to attempt to improve upside assay sensitivity in this dental pain model by: (1) selecting patients with high baseline pain intensity; and (2) closer supervision of outpatients' drug intake and compliance with protocol. Only patients with baseline pain ≥ 50 on a 100 mm visual analogue scale after wisdom tooth surgery were included. Twenty patients were given paracetamol 1000 mg with or without codeine 60 mg in repeated doses in a randomized and double‐blind manner. Intake of the first dose of test medication and its effects were closely supervised, while the two following doses were taken at 3‐h intervals after the patient had left the clinic. Pain intensity was measured with the visual analogue scale for 8 h. Using an affective priming task, it was investigated whether pictures of threatening back‐stressing movements (primes) facilitate (respectively, slow down) the categorisation of subsequent evaluative‐negative (evaluative‐positive) words (targets). In study 1 using 20 pain‐free subjects, the affective priming effect indicated evidence for an implicit negative attitude towards pictures of back‐stressing activities. In study 2 using 30 low back pain patients, a reverse priming effect was found. In line with previous research, it is argued that this reverse priming effect is owing to the evaluative extremity of the primes: patients recognize the possibility that extreme primes will interfere with the categorisation of the targets and overcompensate for this possible effect. management of certain forms of chronic pain that do not respond to conventional pain therapy. Several authors have reported encouraging findings with this technique. Over a 10‐year period in a single centre, 254 patients were subjected to a trial period of spinal cord stimulation with an externalized pulse generator. Two hundred and seventeen of the patients showed satisfactory results justifying permanent implantation of a spinal cord stimulation system. In 1998, an independent physician invited 153 patients (155 pain cases), who still had the system in place and who could be contacted, for an interview. The aim of this study was to evaluate the efficacy of an implanted spinal cord stimulation system in terms of pain relief and quality of life and to assess the accuracy of the patient selection criteria. The results of this study demonstrate a high success rate as evaluated by the patients' own assessments—68% of the patients rated the result of the treatment as excellent to good after an average follow‐up of almost 4 years. In pain therapy, the genetic background influencing the efficacy of opioid therapy is of special interest. Screening for variations in expression of drug metabolizing enzymes has been suggested as a potential tool for improving patient therapy. CYP2D6 genetic variability is supposed to be a major factor of adverse drug reaction, possibly influencing hospital stay and total costs. Further candidate genes involved in pain perception, pain processing and pain management like opioid receptors, transporters and other targets of pharmacotherapy are under investigation. Rather, pain is best represented by a continuum of “widespreadness” from localized pain to pain spread across the body. The objective of this paper was to describe the number of pain sites (NPS) reported in a population study and its association with demographic, lifestyle, and health‐related factors. Using a cross‐sectional design, the Standardized Nordic Questionnaire was used to measure musculoskeletal pain among seven age groups in Ullensaker, Norway (\u003Cjats:italic>n\u003C\u002Fjats:italic>=2926). Results showed that women reported a higher mean NPS than men. A higher NPS was also found for individuals who were separated or divorced, undergoing rehabilitation, or who had a disability pension. Additionally, greater NPS was reported by smokers, individuals with less physical activity, and a higher BMI. A strong linear relationship was found between NPS and reduction in overall health, sleep quality, and psychological health. Results from a multivariate linear regression analysis showed that overall health, sleep quality, and gender demonstrated the strongest associations with increasing NPS, accounting for 31.4% of the variance.
