This has recently been underscored by clinical trials demonstrating that small molecules (fosfamatinib, ibrutinib, idelalisib) inhibiting \u003Cjats:styled-content style=\"fixed-case\">BCR\u003C\u002Fjats:styled-content>‐associated kinases (\u003Cjats:styled-content style=\"fixed-case\">SYK\u003C\u002Fjats:styled-content>,\u003Cjats:styled-content style=\"fixed-case\"> BTK\u003C\u002Fjats:styled-content>,\u003Cjats:styled-content style=\"fixed-case\"> PI\u003C\u002Fjats:styled-content>3K) have an encouraging clinical effect. Here we describe the current knowledge of the specific aspects of \u003Cjats:styled-content style=\"fixed-case\">BCR\u003C\u002Fjats:styled-content> signalling in diffuse large B‐cell lymphoma (\u003Cjats:styled-content style=\"fixed-case\">DLBCL\u003C\u002Fjats:styled-content>), follicular lymphoma, chronic lymphocytic leukaemia (\u003Cjats:styled-content style=\"fixed-case\">CLL\u003C\u002Fjats:styled-content>) and normal B cells. Multiple factors can contribute to \u003Cjats:styled-content style=\"fixed-case\">BCR\u003C\u002Fjats:styled-content> pathway (dys)regulation in these malignancies and the activation of ‘chronic’ or ‘tonic’ \u003Cjats:styled-content style=\"fixed-case\">BCR\u003C\u002Fjats:styled-content> signalling. Iron deficiency anemia (IDA) and beta‐thalassemia trait (B‐TT) are the most common causes of hypochromic microcytic anemias. Many indices have been defined to quickly discriminate these similar entities via parameters obtained from automated blood count analyzers. The purpose of the study was to evaluate the predictive value of these indices in differential diagnosis of IDA and B‐TT in adult cases.\u003C\u002Fjats:p>\u003Cjats:p>\u003Cjats:bold>Methods: \u003C\u002Fjats:bold> This study consists of 45 IDA cases, 36 women and nine men, whose mean age is 33.87 ± 11.59 (mean ± SD) (range 17–57 yr) and 66 B‐TT cases, 41 women and 25 men, whose mean age is 33.26 ± 13.36 (mean ± SD) (range 14–74 yr). IDA cases with Hb value &lt;8.7 g\u002FdL have been excluded because these cases are not confused with B‐TT cases in practice. Patient groups have been evaluated according to red blood cell (RBC), red blood cell distribution width (RDW), Mentzer index, Shine and Lal indices, England and Fraser indices, Srivastava index, Green and King indices, RDW index and Ricerca index. Folate‐deficient cells exhibit a two‐fold increase in thymidine kinase and thymidylate synthase activities. These increased activities were reduced to those of folate‐replete cells by co‐culture of folate‐deficient cells with thymidine. Folate deficiency was accompanied by reduced cellular levels of thymidine triphosphate (TTP) and deoxyguanosine triphosphate (dGTP). Exogenous deoxyuridine produced no increase in the reduced levels of TTP of folate‐deficient cells but effected a two‐fold increase in cellular deoxycytidine triphosphate. Exogenous thymidine increased the reduced TTP levels of folate‐deficient cells and corrected the reduced dGTP level; the increase in cellular TTP accompanying exogenous thymidine was more pronounced in folate‐deficient cells. Unfortunately there is not much material published concerning the incidence of these conditions. We present epidemiologic data on MDS based on case registration of patients in a well‐defined population and as a comparison similar data on acute myeloid leukaemia (AML). Between the years 1978–1992 we registered 120 cases of MDS and 146 cases of AML. The median age for all the MDS cases was 74.1 years for men and 78.2 years for women. Among haematologists there is a suspicion that the incidence of MDS is rising. Our study does not support this opinion. We have divided the study period into 5‐year periods and the crude incidence has been 3.2, 4.1 and 3.5\u002F100 000\u002Fyear for each period. In the age group over 70 years MDS was more frequent than AML and in the last 5‐year period the incidence was 15.0\u002F100 000\u002Fyear for MDS compared to 10.2\u002F100 000\u002Fyear for AML. A case of type II recessive congenital methaemoglobinaemia (RCM) observed in a Lebanese subject with a novel mutation in NADH‐cytochrome b5 reductase gene is described. A homozygous mutation CAC to AA identified at Thr 295 with an out‐of‐frame 1‐bp deletion leads to a frameshift with translational read‐through of the natural stop codon. The molecular mechanism is demonstrated by an \u003Cjats:italic>in vitro\u003C\u002Fjats:italic> translation study. The model of mutated cytochrome b5 reductase protein possessing 46 additional amino acids was obtained by homology modelling. The mutation causes an alteration of hydrophobicity in the carboxyl‐terminal portion, resulting in the conformation being drastically disturbed by the presence of 46 supplementary amino acids. The identical mutation was found in the heterozygous state in the patient's parents and sister. One is a novel mutation 647T→C which leads to substitution of isoleucine by threonine at position 215 (I215T). This maternal mutation was found in several family members. A previously known mutation, 757G→A, leads to the replacement of valine by methionine at position 252 (V252M). The latter mutation was found also in the father and one of the two brothers. The effects of these mutations were analysed on a model of the human b5R protein obtained by homology modelling. Although both amino acid substitutions are located in the NADH‐binding domain, the whole protein structure, especially the region between the flavin adenine dinucleotide and NADH‐binding domains, is disturbed. The structural changes in the I215T mutant are less prominent than those in the V252M mutant. The permeability of red blood cells (RBCs) to thiol containing compounds, reduced glutathione (GSH) and N‐acetyl cysteine (NAC), has been studied in control adult and neonatal cells and after oxidative stress. NAC penetrates the cell membrane easily while GSH hardly permeates. We measured their capacity to enhance intracellular non‐protein thiols (NPSH), after inducing damage to the membrane by formation of defects. Diamide, phenazine methosulfate (PMS) and t‐butyl hydroperoxide (BHP) were chosen as exogenous oxidants, each inducing damage by a different mechanism. Our data indicate that although neonatal cells are more sensitive to oxidative stress, only membrane damage induced by diamide, renders adult and neonatal cells permeable to GSH. intensive transfusions and deferoxamine chelation therapy and who had good compliance with this regimen. \u003Cjats:italic>Methods and Results\u003C\u002Fjats:italic>: We assessed clinically and echocardiographically 202 thalassemia major patients aged 27.3 ± 6.3 yr and 75 age and sex‐matched healthy controls. Overt cardiac disease was encountered in 14 patients (6.9%), including 5 (2.5%) with congestive heart failure, aged 26–37 yr, and 9 with systolic left ventricular (LV) dysfunction, aged 23–37 yr. Ten patients (5.0%) had a history of pericarditis. Left atrial and LV diameters, LV mass and cardiac output were significantly higher in patients than in controls, while peripheral resistance and LV afterload were significantly lower. Relative LV wall thickness did not differ between patients and controls, but it was significantly lower in patients with overt cardiac disease compared to those without (\u003Cjats:italic>P\u003C\u002Fjats:italic> &lt; 0.05). Restrictive LV filling was observed in 37.6% of patients and was significantly more frequent in cases with overt cardiac disease (\u003Cjats:italic>P\u003C\u002Fjats:italic> &lt; 0.01). Pulmonary hypertension was practically absent.
