2 (COX‐2: official gene symbol – PTGS2) has long been regarded as playing a pivotal role in the pathogenesis of airway inflammation in respiratory diseases including asthma. COX‐2 can be rapidly and robustly expressed in response to a diverse range of pro‐inflammatory cytokines and mediators. Thus, increased levels of COX‐2 protein and prostanoid metabolites serve as key contributors to pathobiology in respiratory diseases typified by dysregulated inflammation. But COX‐2 products may not be all bad: prostanoids can exert anti‐inflammatory\u002Fbronchoprotective functions in airways in addition to their pro‐inflammatory actions. Herein, we outline COX‐2 regulation and review the diverse stimuli known to induce COX‐2 in the context of airway inflammation. We discuss some of the positive and negative effects that COX‐2\u002Fprostanoids can exert in \u003Cjats:italic>in vitro\u003C\u002Fjats:italic> and \u003Cjats:italic>in vivo\u003C\u002Fjats:italic> models of airway inflammation, and suggest that inhibiting COX‐2 expression to repress airway inflammation may be too blunt an approach; because although it might reduce the unwanted effects of COX‐2 activation, it may also negate the positive effects. Evidence suggests that prostanoids produced via COX‐2 upregulation show diverse actions (and herein we focus on prostaglandin E\u003Cjats:sub>2\u003C\u002Fjats:sub> as a key example); these can be either beneficial or deleterious and their impact on respiratory disease can be dictated by local concentration and specific interaction with individual receptors. We propose that understanding the regulation of COX‐2 expression and associated receptor‐mediated functional outcomes may reveal number of critical steps amenable to pharmacological intervention. Major peanut allergens Ara h1, Ara h2 and Ara h3 were purified from peanuts using ion‐exchange chromatography. IgE immunoblotting was performed and IgE‐cross‐linking capacity was examined by measuring histamine release (HR) after incubating patient basophils as well as passively sensitized basophils with several dilutions of the allergens. Intracutaneous tests (ICTs) using 10‐fold dilution steps of the purified allergens and crude peanut extract were performed.\u003C\u002Fjats:p>\u003Cjats:p>\u003Cjats:bold>Results\n\t\t\t\t\t\u003C\u002Fjats:bold> Ara h2 was recognized most frequently (26 out of 32) in all tests and induced both positive skin tests and basophil degranulation at low concentrations, whereas Ara h1 and Ara h3 were recognized less frequently and reacted only at 100‐fold higher concentrations as analysed with HR and intracutaneous testing (ICT). IgG4 is a very dynamic antibody: the antibody is involved in a continuous process of half‐molecules (i.e. a heavy and attached light‐chain) exchange. This process, also referred to as ‘Fab‐arm exchange’, results usually in asymmetric antibodies with two different antigen‐combining sites. While these antibodies are hetero‐ bivalent, they will behave as monovalent antibodies in most situations. Another aspect of IgG4, still poorly understood, is its tendency to mimic IgG rheumatoid factor (RF) activity by interacting with IgG on a solid support. In contrast to conventional RF, which binds via its variable domains, the activity of IgG4 is located in its constant domains. This is potentially a source of false positives in IgG4 antibody assay results. Because regulation of IgG4 production is dependent on help by T‐helper type 2 (Th2) cells, the IgG4 response is largely restricted to non‐microbial antigens. This Th2‐dependency associates the IgG4 and IgE responses. Another typical feature in the immune regulation of IgG4 is its tendency to appear only after prolonged immunization. In the context of IgE‐mediated allergy, the appearance of IgG4 antibodies is usually associated with a decrease in symptoms. This is likely to be due, at least in part, to an allergen‐blocking effect at the mast cell level and\u002For at the level of the antigen‐presenting cell (preventing IgE‐facilitated activation of T cells). In addition, the favourable association reflects the enhanced production of IL‐10 and other anti‐inflammatory cytokines, which drive the production of IgG4. While in general, IgG4 is being associated with non‐activating characteristics, in some situations IgG4 antibodies have an association with pathology. Two striking examples are pemphigoid diseases and sclerosing diseases such as autoimmune pancreatitis. The mechanistic basis for the association of IgG4 with these diseases is still enigmatic. However, the association with sclerosing diseases may reflect an excessive production of anti‐inflammatory cytokines triggering an overwhelming expansion of IgG4‐producing plasma cells. The bottom line for allergy diagnosis: IgG4 by itself is unlikely to be a cause of allergic symptoms. In general, the presence of allergen‐specific IgG4 indicates that anti‐inflammatory, tolerance‐inducing mechanisms have been activated. Another aim was to relate the results to self‐reported frequency of allergy and asthma symptoms among the children, to their perception of the school environment.\u003C\u002Fjats:p>\u003Cjats:p>\u003Cjats:bold>Methods\u003C\u002Fjats:bold> Among all compulsory schools (\u003Cjats:italic>n\u003C\u002Fjats:italic> = 257) in the Stockholm suburban area, 35 classrooms (five with implemented allergy prevention measures, seven with additional cleaning and 23 with normal cleaning routines) were chosen for allergen‐sampling. Dust collectors (two models), Petri dishes, person‐carried pumps and intranasal samplers were used simultaneously. All children (\u003Cjats:italic>n\u003C\u002Fjats:italic> = 829) received a self‐administered questionnaire which included questions about home and school environment, allergic disease, asthma symptoms and pet contact.\u003C\u002Fjats:p>\u003Cjats:p>\u003Cjats:bold>Results\u003C\u002Fjats:bold> The correlation between sampling methods was generally poor. Furthermore, there was no significant difference in allergen levels between allergy prevention and allergen avoidance classes compared to conventional classes. Median levels were generally, but not significantly, lower in classes with few cat owners, compared to classes with many cat owners. Children in allergy prevention classes were more satisfied with the indoor air quality and cleaning than children attending classes with fewer or no allergy prevention measures (\u003Cjats:italic>P\u003C\u002Fjats:italic> &lt; 0.0001). Nine per cent of all children reported allergic symptoms while at school.\u003C\u002Fjats:p>\u003Cjats:p>\u003Cjats:bold>Conclusion\u003C\u002Fjats:bold> The lack of correlation between sampling methods used simultaneously demonstrates the difficulty in assessing allergen levels in schools and similar environments. We have tested the effect of exposure conditions and level of activity on measurements of the personal exposure.\u003C\u002Fjats:p>\u003C\u002Fjats:sec>\u003Cjats:sec>\u003Cjats:title>Method\u003C\u002Fjats:title>\u003Cjats:p>Personal Air Samplers (PAS) and Nasal Air Samplers (NAS) were employed. NAS are fitted just inside the nose and collect inhaled particles by impaction, while the PAS use a pump‐operated filter with constant air flow (2 L\u002Fmin). Thirty‐three subjects (adults and children) used both NAS and PAS simultaneously for four one hour periods during which they performed activities or rested, both inside and outside their homes. Samples were analysed by light microscopy. \u003Cjats:italic>Alternaria \u003C\u002Fjats:italic>spores, \u003Cjats:italic>Cladosporium \u003C\u002Fjats:italic>spores, grass pollen and nongrass pollen were counted.\u003C\u002Fjats:p>\u003C\u002Fjats:sec>\u003Cjats:sec>\u003Cjats:title>Results\u003C\u002Fjats:title>\u003Cjats:p>Both samplers detected substantial variation in exposure between subjects. Between members of the same household, the intrahouse correlation coefficient ranged from &lt; 0 − 0.38. Levels of pollen grains and fungal spores inhaled were higher during periods of activity than during rest, and higher while subjects were outdoors than indoors. During the active outdoor period, the number of \u003Cjats:italic>Alternaria\u003C\u002Fjats:italic> spores inhaled ranged from 4 to 794 (median 11) spores\u002Fhr, \u003Cjats:italic>Cladosporium\u003C\u002Fjats:italic> from 0 to 396 (median 4) spores\u002Fhr, grass pollen from 0 to 81 (median 1) grains\u002Fhr and nongrass pollen from 0 to 72 (median 5) grains\u002Fhr.\u003C\u002Fjats:p>\u003C\u002Fjats:sec>\u003Cjats:sec>\u003Cjats:title>Conclusion\u003C\u002Fjats:title>\u003Cjats:p>This is the first study to quantify individual inhaled levels of allergenic fungal spores and pollen under normal domestic circumstances. Exposure can be substantial and highly variable between individuals. The IgE‐immunoblots were analysed by densitometry and integration of the densitogram peaks. A standardized reference disc system allowed parallel analysis of several different immunoblotting experiments. Altogether 105 patients had \u003Cjats:italic>C. albcans\u003C\u002Fjats:italic> specific IgE antibodies in immunoblotting. The IgE‐response against \u003Cjats:italic>C. albicans\u003C\u002Fjats:italic> was extremely polyspecific and was directed against 42 different bands. The most important IgE binding bands were 46 kDa and 27 kDa bands against which 44 (42%) and 29 (28%) patients, respectively, had IgE responses. A combination of IgE‐immunoblotting and densitometry was found practical in analysis of large number of patient sera allowing a profound and accurate analysis and characterization of C. \u003Cjats:italic>albicans\u003C\u002Fjats:italic> allergen extract.
