We determined the association with cardiac biomarkers (peak \u003Cjats:styled-content style=\"fixed-case\">CK‐MB\u003C\u002Fjats:styled-content>, peak troponin T, N‐terminal pro‐brain natriuretic peptide), clinical and angiographic characteristics with infarct size, and \u003Cjats:styled-content style=\"fixed-case\">LVEF\u003C\u002Fjats:styled-content>, followed by association with mortality in 1120 \u003Cjats:styled-content style=\"fixed-case\">STEMI\u003C\u002Fjats:styled-content> patients.\u003C\u002Fjats:p>\u003C\u002Fjats:sec>\u003Cjats:sec>\u003Cjats:title>Results\u003C\u002Fjats:title>\u003Cjats:p>Peak \u003Cjats:styled-content style=\"fixed-case\">CK‐MB\u003C\u002Fjats:styled-content> was the strongest predictor for infarct size (\u003Cjats:italic>P\u003C\u002Fjats:italic>&lt;0.001, \u003Cjats:italic>R\u003C\u002Fjats:italic>\n\u003Cjats:sup>2\u003C\u002Fjats:sup>=0.60) and LVEF (\u003Cjats:italic>P\u003C\u002Fjats:italic>&lt;0.001, \u003Cjats:italic>R\u003C\u002Fjats:italic>\n\u003Cjats:sup>2\u003C\u002Fjats:sup>=0.40). The additional value of clinical and angiographic characteristics was limited. The optimal peak CK‐MB cutpoints, for differentiation among small (&lt;10% of the left ventricle), moderate (≥10%–&lt;30%), and large infarct size (≥30%), were 210 U\u002FL and 380 U\u002FL, respectively. These cutpoints were associated with 90‐day mortality; the hazard ratio for moderate infarct was 2.99 (95% confidence interval [CI]: 1.51‐5.93, \u003Cjats:italic>P\u003C\u002Fjats:italic>=0.002) and for large infarct 6.53 (95% CI: 3.63‐11.76, \u003Cjats:italic>P\u003C\u002Fjats:italic>&lt;0.001).\u003C\u002Fjats:p>\u003C\u002Fjats:sec>\u003Cjats:sec>\u003Cjats:title>Conclusions\u003C\u002Fjats:title>\u003Cjats:p>Classical peak CK‐MB measured during hospitalization for STEMI was superior to other clinical and angiographic characteristics in predicting CMRI‐defined infarct size and LVEF, and should be included and validated in future multimarker studies. fibrillation (AF) is an epidemic, affecting 1% to 1.5% of the population in the developed world. Projected data from the population‐based studies suggest that the prevalence of AF will grow at least 3‐fold by 2050. The health and economic burden imposed by AF and AF‐related morbidity is enormous.\u003C\u002Fjats:p>\u003Cjats:p>Atrial fibrillation has a multiplicity of causes ranging from genetic to degenerative, but hypertension and heart failure are the commonest and epidemiologically most prevalent conditions associated with AF as both have been shown to create an arrhythmogenic substrate. Several theories emerged regarding the mechanism of AF, which can be combined into two groups: the single focus hypothesis and the multiple sources hypothesis. Several lines of evidence point to the relevance of both hypotheses to the mechanism of AF, probably with a different degree of involvement depending on the variety of AF (paroxysmal or persistent). Sustained AF alters electrophysiological and structural properties of the atrial myocardium such that the atria become more susceptible to the initiation and maintenance of the arrhythmia, a process known as atrial remodeling. Angiotensin II has been recognized as a key element in atrial remodeling in association with AF opening the possibility of exploitation of “upstream” therapies to prevent or delay atrial remodeling.\u003C\u002Fjats:p>\u003Cjats:p>The clinical significance of AF lies predominantly in a 5‐fold increased risk of stroke. The limitations of warfarin prompted the development of new antithrombotic drugs, which include anticoagulants, such as direct oral thrombin inhibitors (dabigatran) and factor Xa inhibitors (rivaroxaban, apixaban). Novel mechanical approaches for the prevention of cardioembolic stroke have recently been evaluated: percutaneous left atrial appendage occluders, minimally invasive surgical isolation of the left atrial appendage, and implantation of carotid filtering devices. Patients with pre‐existing cardiovascular disease are at higher risk of morbidity and mortality. The virus may affect the heart directly and indirectly with clinical syndromes of acute myocardial injury, myocarditis, acute coronary syndromes, heart failure, arrhythmias, and venous thromboembolism. Some therapeutics under investigation for COVID‐19 may also have adverse cardiac effects. The involvement of the RAAS system in viral entry makes it pertinent to consider the effects of medications that modulate the system. Median (interquartile range) coronary artery calcium score (CACS) was 34 (0–100). Over a median follow‐up of 4.4 (3.4–5.1) years, there were 53 MACE (10%). Patients experiencing MACE had higher CACS, incidence of carotid disease, presence of CAD ≥50%, and remodeled plaque as compared with patients without MACE. At multivariable analyses, presence of CAD ≥50% (HR: 5.14, 95% CI: 2.1–12.4) and percentage of segments with remodeled plaque (HR: 1.04, 95% CI: 1.03–1.06) independently predicted MACE (\u003Cjats:italic>P\u003C\u002Fjats:italic> &lt; 0.001). Models adding CAD ≥50% or percentage of segments with remodeled plaque resulted in higher discrimination and reclassification ability compared with a model based on 10‐year FRS, carotid disease, and CACS. obstructive pulmonary disease (COPD) is a progressive and debilitating disease. Cor pulmonale, characterized by right ventricular (RV) failure, can severely influence prognosis in these patients. Hence, early recognition might be important for tailoring therapy. An old biomarker, CA‐125, seems to be associated with the right heart. We aimed to show the relationship between CA‐125 levels and RV failure in patients with COPD.\u003C\u002Fjats:p>\u003C\u002Fjats:sec>\u003Cjats:sec>\u003Cjats:title>Hypothesis:\u003C\u002Fjats:title>\u003Cjats:p>CA‐125 might be a useful biomarker in identification of RV failure in patients with COPD.\u003C\u002Fjats:p>\u003C\u002Fjats:sec>\u003Cjats:sec>\u003Cjats:title>Methods:\u003C\u002Fjats:title>\u003Cjats:p>Forty patients with recent exacerbation of COPD were enrolled into the study. Another 40 age‐ and sex‐matched individuals were enrolled for comparison. Levels of CA‐125 were measured in the patients during the hospital stay. The COPD patients underwent echocardiographic study on the same day. Right‐ventricular parameters were evaluated, and RV failure was identified via transthoracic echocardiography.\u003C\u002Fjats:p>\u003C\u002Fjats:sec>\u003Cjats:sec>\u003Cjats:title>Results:\u003C\u002Fjats:title>\u003Cjats:p>Patients with COPD had significantly higher CA‐125 levels compared with controls (median 33.94 U\u002FmL vs 9.76 U\u002FmL, respectively; \u003Cjats:italic>P &lt;\u003C\u002Fjats:italic> 0.001). Using DTI, the time intervals from the beginning of P‐wave to the onset of atrioventricular ring motion related to atrial contraction were measured.\u003C\u002Fjats:p>\u003C\u002Fjats:sec>\u003Cjats:sec>\u003Cjats:title>Results\u003C\u002Fjats:title>\u003Cjats:p>Atrial electromechanical coupling at the interventricular septum atrioventricular annulus (P‐A1), left lateral mitral annulus (P‐A2) and right lateral tricuspid annulus (P‐A3) in PAF group were significantly longer than those in control (p &lt; 0.001). The difference between P‐A2 and P‐A1 (T1), P‐A2 and P‐A3 (T3) in PAF group were greater than those in control before age correction (p &lt; 0.05). The linear regression analysis showed that the duration of PAF episodes and age were the greatest influential factors of P‐A1 (r = 0.564). Left atrial anterior‐posterior dimension (LAD) and age were the greatest influential factors of P‐A2 (r = 0.459). The LAD was the greatest influential factors of T1 and T3 (r = 0.408, 0.542).\u003C\u002Fjats:p>\u003C\u002Fjats:sec>\u003Cjats:sec>\u003Cjats:title>Conclusions\u003C\u002Fjats:title>\u003Cjats:p>The atrial electromechanical coupling was significantly longer and the dispersion of P‐A at left lateral mitral annulus was greater in PAF patients. The prolongation of P‐A may be related to left atrial enlargement, long episodes of PAF and aging and the dispersion of P‐A at left lateral mitral annulus to LAD. designation of atherosclerosis as a chronic inflammatory process represents an exciting and logical paradigm shift for cardiologists. Plasma concentrations of interleukin‐6 (IL‐6) and its hepatic by‐product C‐reactive protein (CRP) appear to reflect the intensity of occult plaque inflammation and by inference may determine vulnerability to rupture. Indeed, circulating IL‐6 levels are elevated in patients with acute myocardial infarction (AMI), and also in patients with unstable, but not with stable angina. Coronary sinus IL‐6 concentrations are also increased after percutaneous coronary intervention (PCI), and late restenosis correlates with an increase in IL‐6 concentration after the procedure, indicating that IL‐6 expression may be not only related to instability of atheromatous plaques, but also to the formation of restenotic lesions after PCI. These observations suggest the advantage of screening for circulating IL‐6 concentration and the use of anti‐inflammatory treatment for those thought be at high risk to reduce the risk of future
