Une étude prospective transversale à visée descriptive et analytique d’un échantillon de 393 sujets âgés de plus de 5 ans résidant dans le village de Gami depuis plus de deux ans et ayant été traités par l’ivermectine lors de la dernière campagne qui avait eu lieu 10 mois avant. Les données épidémiologiques, cliniques et parasitologiques recueillies ont été comparées aux résultats des enquêtes antérieures dans le village. Les paramètres se sont nettement améliorés pendant les 20 années (1990–2010) notamment la prévalence de la microfilarodermie (88 % en 1990 contre 19 % en 2010), la densité microfilarienne moyenne (54 contre 0,7 microfilaires par biopsie), la charge microfilarienne de communauté (CMFL) (39 contre 0,67 microfilaires par biopsie), l’indice de Knüttgen (qui s’est déplacé de la tranche d’âge de 5–9 ans à celle de plus de 45 ans dès 1998), la prévalence des nodules (36 % contre 8 %), celles des lésions oculaires onchocerquiennes (28 % contre 2 %) et des lésions oculaires graves (16 % contre 1,3 %), le taux de cécité (9 % contre 0,8 %), la prévalence de la perte de vision importante (3 % contre 0,0 %) et celle des lésions oculaires chez les enfants de 6–10 ans (6 % contre 0,3 %). Ces données permettent de parler de contrôle et non d’élimination de l’onchocercose dans le groupement villageois de Gami. The present study aimed at evaluating the cervical cancer screening rate between 2006 and 2011 by age and for a 3-year period in French Guiana. All pap smears realised in French Guiana were analysed in two laboratories allowing exhaustive review of screening data. The screening rate was estimated at about 54% from 2006 to 2011, with a statistical difference between coastal and rural area (56.3% versus 18.7%). Ce genre comprend plusieurs espèces, dont la plus répandue est Gnathostoma spinigerum. Le cycle parasitaire est complexe et comporte au moins 3 hôtes avec la possibilité d’hôtes paraténiques. Le premier hôte intermédiaire est un crustacé (cyclops), le deuxième un poisson d’eau douce ou un batracien, tous les vertébrés peuvent être hôte paraténique, en particulier le porc et les oiseaux. L’Homme se contamine en consommant la chair crue ou peu cuite des hôtes intermédiaires ou paraténiques. Le plus souvent il s’agit de poissons d’eau douce, de porcs, mais aussi d’autres hôtes intermédiaires (serpents, anguilles…) contenant la larve infestante au troisième stade (L3). La contamination par pénétration transcutanée des larves L3 est possible, le temps moyen de pénétration étant de 30 minutes avec un minimum de 6 minutes. Chez l’Homme comme chez les hôtes paraténiques le cycle est bloqué, c’est une impasse parasitaire: la larve L3 « erre » dans les tissus, responsable d’un syndrome de larva migrans cutanée et\u002Fou viscérale. Nous rapportons le premier cas documenté de gnathostomose intra-oculaire survenu au Cambodge. Il s’agit d’un homme de 37 ans présentant des épisodes de cécité de l’oeil gauche, avec douleurs, iritis, larmoiement et oedème. L’examen à la lampe à fente a permis de mettre en évidence une larve de nématode, dont l’aspect était très évocateur de Gnathostoma spinigerum: tête typique en forme d’ampoule avec cinq rangées de petits crochets et fines épines cuticulaires sur sa surface corporelle. La mise en évidence de la larve a permis de faire le diagnostic de certitude. Lorsque la larve n’est pas identifiée, le diagnostic repose le plus souvent sur un faisceau d’arguments épidémiologiques, cliniques et biologiques (hyperéosinophilie, sérodiagnostic positif). Il s’agit du premier cas intraoculaire signalé au Cambodge. Cette pathologie ne doit pas être ignorée car la gnathostomose est une des causes de méningo-encéphalite à éosinophiles. Les investigations ont eu pour objectif d’identifier les vecteurs et les hôtes intermédiaires des parasitoses liées à l’eau. Les prévalences du paludisme, de la bilharziose et des géohelminthiases ont été évaluées avant la mise en eau du barrage. L’échantillonnage s’est fait par capture des moustiques sur homme, prospections malacologiques au niveau des sites de contact homme-eau, gouttes épaisses (GE) et frottis sanguins (FS), et prélèvements des selles et urines. Deux espèces vectrices du paludisme ont été identifiées: Anopheles gambiae et Anopheles funestus. L’indice sporozoïtique moyen annuel d’ An. gambiae a varié de 3,1 à 4,5 %. An. funestus a présenté un indice sporozoïtique de 1 % dans tous les sites. L’objectif de cette étude est d’étudier les facteurs influençant l’accès au traitement dans une situation de gratuité. Il s’agit d’une étude transversale et analytique portant sur un échantillon de 1 906 femmes âgées de 15 à 49 ans tiré au sort lors de l’enquête nationale sur le paludisme au Sénégal. Les données ont été collectées lors d’un entretien individuel. Le bien-être économique était mesuré à partir des caractéristiques du logement et des biens durables. L’analyse multivariée a été réalisée à l’aide d’une régression logistique. La moyenne d’âge des femmes était de 27,9 ± 5,34; 64,3 % résidaient en milieu rural; 71,8 % des femmes n’avaient reçu aucune instruction. Parmi les femmes étudiées, 23 % étaient dans le quintile le plus pauvre tandis que 16,3 % étaient dans le plus riche. Le traitement préventif intermittent (TPI) était effectué dans 49,3 % des cas. Le TPI était plus souvent réalisé en milieu urbain (OR: 1,45; IC 95 %: [1,17–1,72]). Il augmentait avec le niveau d’instruction avec un OR à 1,5 au primaire et 1,68 au secondaire et plus. La réalisation du TPI augmentait avec le bien-être économique. L’OR variait de 1,44 au deuxième quintile à 2,95 pour les plus riches. La gratuité ne profite pas souvent aux plus pauvres. Ce virus, considéré jusqu’à récemment comme un virus bénin, a révélé des complications jusqu’alors insoupçonnées: manifestations neurologiques graves chez l’adulte et malformations du système nerveux central chez les nouveau-nés de mère atteintes pendant la grossesse. Alors que le continent est plutôt habitué aux épidémies d’arbovirus, les complications suspectées et les nombreuses inconnues de ce dernier arrivant posent de nombreuses questions de santé publique. La Guyane française, département français situé au nord-est du continent sud-américain, associe à la fois des ressources de niveau européen et un climat et des problématiques propres à la région amazonienne et latino-américaine. Nous discutons ici, sur les bases d’une revue de la littérature et de nos expériences de terrain, des enjeux et des stratégies à mettre en oeuvre pour 2016 suite à l’épidémie de Zika dans notre territoire.
