is a ɣ-cyclodextrin containing 8-thiopropionate side chains, which selectively binds to nondepolarizing aminosteroid neuromuscular blocking agents. Here, we report a case who developed deep bradycardia after administration of sugammadex. A 38-year-old-man was scheduled for laparoscopic right hemicolectomy. Besides history of light smoking, he had no other medical/surgical history that included any allergic reactions. At the end of the operation, 200 mg sugammadex was administered to antagonize residual neuromuscular blockade. One minute after the administration of sugammadex, the patient had deep bradycardia (25 beat min−1) and his systolic blood pressure fell below a measurable level. The patient’s blood pressure was restored to 95/55 mmHg and heart rate 110 beats min−1 with the administration of a total dose of ephedrine 10 mg, atropine sulfate 0.5 mg, 0.9% saline 1 L, and 6% hydroxyethylated starch 500 mL over 15 min. Then, he was extubated uneventfully and transferred to the intensive care unit for closer monitoring. According to the current literature as well as the case we presented here, we suggest that physicians who use sugammadex especially in endoscopic/laparoscopic procedures using CO2 should be aware of the possibility of sudden bradycardia and/or cardiac arrest.
Deep bradycardia after sugammadex: is it due to anaphylaxis or by any other unknown mechanism(s) of sugammadex?
Bhavani SS (2018) Severe bradycardia and asystole after sugammadex. This prospective, single-blinded, randomized clinical study included a total of forty newborn babies scheduled to undergo elective circumcision. The babies were randomized into two treatment groups of ultrasound (US)-guided penile block (group I; n = 20) and classical landmark method dorsal penile nerve block (DPNB) (group II; n = 20) group. Face, Legs, Activity, Cry, Consolability (FLACC) score was used to determine the block efficacy and postoperative pain and analgesic requirements. Intraoperative and postoperative FLACC scores, intraoperative analgesic needs, discharge time, complications, first oral intake time, and parent’s satisfaction were also recorded. FLACC scores were higher in group II than in group I during the intraoperative periods. Heart rate was higher in group II than in group I, at the incision, and during the procedure (P < 0.05). The number of patients requiring fentanyl was higher in group II than in group I (P < 0.01). FLACC scores were statistically higher in the landmark group at arrival in the PACU (P < 0.01) and after 30 min up to 2 h (P < 0.01). randomized, double-blind study was designed to compare between sequential combined spinal epidural anesthesia versus epidural volume extension in lower limb surgery as regards hemodynamics, sensory, and motor blocks. In this randomized, double-blind, prospective study, 80 patients scheduled for lower limb surgery were divided into two groups: sequential combined spinal epidural (SCSE) group in which small doses of local anesthetic was injected in epidural space after low-dose spinal anesthesia and epidural volume extension (EVE) group in which 10 ml saline was injected in epidural space after low-dose spinal anesthesia. Hemodynamics, anesthesia readiness time, degree of motor block, time to regression of sensory block, and side effects were measured. Hemodynamic changes were insignificant. Anesthesia readiness time was significantly faster in EVE group. Motor block and sensory block were better in SCSE. Postoperative bupivacaine consumption was statistically insignificant between the two groups. Both SCSE and EVE techniques can preserve hemodynamics after low-dose subarachnoid block and can be used in high-risk elderly patients undergoing orthopedic surgery.
Comparative study between sequential combined spinal epidural anesthesia versus epidural volume extension in lower limb surgery
Bernstein RL, Rosenberg AD. The aim of this study was to clarify the effect of high body mass index (BMI) and other factors on the development of PPCs and the perioperative course. This study retrospectively analyzed patients admitted to the intensive care unit (ICU) after undergoing elective upper abdominal surgery at our hospital between December 2015 and December 2018. The primary endpoint was the effect of BMI on the development of PPCs, and the secondary endpoints were the effect of BMI on length of ICU stay, ICU readmission, length of hospital stay, and death within 30 days of surgery. A total of 231 patients were included in this study, 27 of whom had PPCs. BMI was not significantly associated with the development of PPCs. BMI was not significantly associated with length of ICU stay or ICU readmission, whereas higher BMI was significantly associated with shorter hospital stays. The only death within 30 days of surgery occurred in a patient who developed PPCs and died while in the ICU. anesthesia is efficient but of limited duration. Intrathecal dexmedetomidine prolongs the sensory and motor blockade of bupivacaine. Dexmedetomidine-opioids combination displayed a clinically controversial interaction. Our proposal is that fentanyl may augment the block characteristics of dexmedetomidine using proper doses. This is a randomized double-blinded study. The included patients were scheduled for orthopedic procedures expected to extend more than 4 h. Patients were allocated into two groups each of 23. Group D received intrathecal bupivacaine 20 mg 0.5% + dexmedetomidine 10 μg. Group DF received bupivacaine 20 mg 0.5% + dexmedetomidine 10 μg + fentanyl 25 μg. The spinal block characteristics and adverse effects were determined. Data were compared by t test, Mann-Whitney, and chi-square tests as appropriate. There was no significant difference between the two groups as regards spinal block and hemodynamic characteristics. The addition of fentanyl provided earlier time to T10 sensory block, lower midazolam and ephedrine utilization, but occasional mild itching. Postoperatively, the time to the first analgesic request, morphine consumption, and patient’s satisfaction were not different. The addition of fentanyl does not prolong the sensory and motor block characteristics of dexmedetomidine. In favor of dexmedetomidine-fentanyl combination was the less hypotension and less sedative requirement. Intravenous magnesium sulfate [MgSO4] has been proven to be effective in prevention of post-spinal shivering. However, the risk of intravenous route to develop hypermagnesemia in certain patients encouraged us to do this study in order to investigate the effectiveness of intrathecal MgSO4 as an alternative to the intravenous route in prevention of post-spinal shivering. The enrolled 135 patients were allocated to 3 groups, 45 patients each, groups: intrathecal [T], IV MgSO4 infusion [M], and control [C]. Group T (no. = 45) received 50 mg MgSO4 added to heavy bupivacaine 0.5% intrathecally, while group M (no. = 45) received IV MgSO4 as 50 mg/kg in 100 cc saline within 20 min as a loading dose then infused as 2 mg/kg/h after performing spinal anesthesia. The vital signs, duration and shivering grades, temperature, and adverse effects were recorded. T and M groups showed a significant difference from control as regard the incidence of shivering with 40%, 26.7%, and 64.4% for T, M, and C groups, respectively, patients needed rescue pethidine were 17.7%, 11.1%, and 60% for group T, M, and C, respectively, and shivering duration (min) was 24.86 ± 7.411, 20.47 ± 6.61, and 45 ± 36.2 for groups T, M, and C, respectively with a statistically insignificant difference between group T and M as regards these parameters. Intrathecal MgSO4 has the potentiality to be used as an alternative to IV route regarding prevention of post-spinal shivering. This makes advantage for intrathecal magnesium use in patients at risk for magnesium toxicity.
Magnesium sulfate intravenous infusion versus intrathecal injection for prevention of post-spinal shivering during lower limb fracture surgery: a randomized controlled study
citation_journal_title=Anesth Analg; citation_title=The effect of fentanyl and morphine on neurons in the dorsal raphe nucleus in the rat: an in vitro study; citation_author=ME Alojado, Y Ohta, T Yamamura, O Kemmotsu; citation_volume=78; citation_issue=4; citation_publication_date=1994; citation_pages=726-732; citation_doi=10.1213/00000539-199404000-00019; citation_id=CR1
citation_journal_title=J Anesthesiol Clin Pharmacol; citation_title=Reduction in the incidence of shivering with perioperative dexmedetomidine. study was designed to evaluate the effects of infusion of magnesium sulfate compared to dexmedetomidine on the postoperative analgesic consumption and pain control in patients scheduled for video-assisted thoracoscopic surgeries (VATS). The intraoperative hemodynamics, anesthesia requirements, and recovery profile were also evaluated. The mean arterial pressure (MAP) and heart rate (HR) recordings were significantly lower in group D than in groups C and M. The MAP recordings were significantly lower in group M than in group C with no significant difference as regards the HR recordings between both groups. Intraoperative sevoflurane and fentanyl requirements were significantly lower in groups D and M than in group C and in group D than in group M. The atracurium consumption was significantly lower in group M than in groups C and D. The time to reach modified Aldrete score ≥ 9 was significantly longer in groups D and M than in group C and in group D than in group M. Postoperative Ramsay sedation scores were significantly higher in groups D and M than in group C throughout the PACU stay and in group D than in group M in the 1st h postoperatively. The VAS score recordings were significantly lower in groups D and M than in group C and in group D than in group M except at 24-h postoperative recordings. The postoperative nalbuphine and ketorolac requirements were significantly lower in groups D and M than in group C and in group D than in group M. During VATS, patients who received dexmedetomidine had better hemodynamic stability, less intraoperative anesthetic consumption with better quality of postoperative analgesia, and less postoperative analgesic consumption but longer postoperative anesthesia recovery and higher postoperative sedation scores compared with magnesium sulfate.
Dexmedetomidine versus magnesium sulfate as adjunct to general anesthesia in patients undergoing video-assisted thoracoscopy
Aboushanab OH, El-Shaarawy AM, Omar AM et al (2011) A comparative study between magnesium sulphate and dexmedetomidine for deliberate hypotension during middle ear surgery.
